摘 要:基于Meta分析评价百令胶囊对慢性阻塞性肺疾病(Chronic Obstructive Pulmonary Disease,COPD)稳定期患者免疫、炎症及氧化应激指标的影响。方法检索Cochrane Library、PubMed、EMbase、中国知网(CNKI)、中国生物医学文献数据库(CBM)、万方数据库(Wanfang Data)和维普数据库(VIP),根据纳入和排除标准筛选出百令胶囊治疗COPD稳定期患者的随机对照试验(randomized controlled trials,RCT),对入选研究进行质量评价,采用Rev Man 5.3软件对研究结局数据进行Meta分析。结果最终纳入21项RCT,共2086例患者。Meta分析结果显示,百令胶囊可以改善稳定期COPD患者免疫指标,包括CD3+[MD=6.48,95%CI(3.82,9.15),P<0.00001]、CD4+[MD=6.93,95%CI(2.14,11.71),P=0.005]、CD8+[MD=-5.38,95%CI(-9.33,-1.44),P=0.007]、CD4+/CD8+[MD=0.33,95%CI(0.15,0.51),P=0.0003]、Th17%[MD=-1.29,95%CI(-1.49,-1.09),P<0.00001]、Treg%[MD=0.18,95%CI(0.08,0.28),P=0.0003]、Th17/Treg[MD=-0.07,95%CI(-0.08,-0.06),P<0.00001];炎症指标,含白细胞介素-8(IL-8)[MD=-1.76,95%CI(-3.01,-0.51),P=0.006]、转化生长因子-β1(TGF-β1)[MD=-2.38,95%CI(-3.68,-1.07),P=0.0004]、肿瘤坏死因子-α(TNF-α)[MD=-0.87,95%CI(-1.19,-0.54),P<0.00001];氧化应激指标,包括超氧化物歧化酶(SOD)[MD=2.04,95%CI(0.79,3.29),P=0.001]、丙二醛(MDA)[MD=-2.39,95%CI(-3.98,-0.81),P=0.003],差异均有统计学意义。结论百令胶囊可以改善COPD稳定期患者的免疫、炎症及氧化应激指标。Objective To evaluate the effect of Bailing Capsules on indices of immunity,inflammation and oxidative stress of patients with stable chronic obstructive pulmonary disease(COPD)by using meta-analysis.Methods The Cochrane Library,PubMed,EMbase,CNKI,CBM,VIP databases were searched to collect randomized controlled trials(RCTs)of Bailing Capsules in treating stable COPD according to the inclusive and exclusive criteria.The quality of the trials were evaluated and meta-analysis was performed by RevMan 5.3 software.Results A total of 21 RCTs involving 2086 patients were included.The results of meta-analysis showed that:for stable COPD patients,Bailing Capsules can improve the immune indices including CD3+(MD=6.48,95%CI 3.82 to 9.15,P<0.00001),CD4+(MD=6.93,95%CI 2.14 to 11.71,P=0.005),CD8+(MD=-5.38,95%CI-9.33 to-1.44,P=0.007),CD4+/CD8+(MD=0.33,95%CI 0.15 to 0.51,P=0.0003),Th17%(MD=-1.29,95%CI-1.49 to-1.09,P<0.00001),Treg%(MD=0.18,95%CI 0.08 to 0.28,P=0.0003),Th17/Treg(MD=-0.07,95%CI-0.08 to-0.06,P<0.00001),the inflammatory indices including IL-8(MD=-1.76,95%CI-3.01 to-0.51,P=0.006),TGF-β1(MD=-2.38,95%CI-3.68 to-1.07,P=0.0004),TNF-α(MD=-0.87,95%CI-1.19 to-0.54,P<0.00001),the oxidative stress indices including SOD(MD=2.04,95%CI 0.79 to 3.29,P=0.001),MDA(MD=-2.39,95%CI-3.98 to-0.81,P=0.003),and the difference was statistical significance.Conclusion Bailing Capsules can improve the indices of immunity,inflammation and oxidative stress of patients with stable COPD.
