清热解毒方对实验性胃溃疡大鼠模型的作用及对胃黏膜中表皮生长因子表达的调节机制

摘　　要：目的探究清热解毒方对胃溃疡大鼠模型的保护作用及其对胃黏膜中表皮生长因子表达的调节。方法选择50只SD雄性大鼠,用乙酸复制实验性胃溃疡大鼠模型,随机分为对照组、模型组、兰索拉唑组、清热解毒方组和联合给药组,对照组和模型组灌胃蒸馏水,兰索拉唑组灌胃兰索拉唑,清热解毒方灌胃清热解毒方,联合给药组灌胃兰索拉唑和清热解毒方。比较各组胃溃疡指数、胃溃疡抑制率、胃液pH值、胃蛋白酶活性、胃组织病理变化、血清中EGF和EGFR水平。结果清热解毒方组和兰索拉唑组胃溃疡指数和胃蛋白酶活性均显著大于联合给药组,差异有统计学意义(P<0.05);清热解毒方组和兰索拉唑组大鼠的胃溃疡抑制率、胃液pH值、血清中EGF和EGFR水平均显著小于联合给药组,差异有统计学意义(P<0.05);清热解毒方组和兰索拉唑组胃溃疡指数、胃蛋白酶活性、胃溃疡抑制率、胃液pH值、血清中EGF和EGFR水平均没有显著性差异(P>0.05);兰索拉唑组和清热解毒方组大鼠胃黏膜损伤程度显著降低,胃黏膜上皮变性坏死以及溃疡症状均小于模型组,胃黏膜上水肿、出血、炎症等症状轻中度;联合给药组大鼠胃黏膜损伤显著改善,与对照组大鼠胃组织病理变化基本相同。结论清热解毒方可以有效改善乙酸致实验性胃溃疡大鼠的胃溃疡症状,但联合应用西药与清热解毒方时,则能收到更加有效的治疗效果。清热解毒方促进胃溃疡修复过程的机制可能为促进EGF和EGFR表达。Objective To investigate the protective effect of Qingre Jiedu Formula on experimental gastric ulcer rat model and its regulation on the expression of epidermal growth factor in gastric mucosa.Methods A total of 50 SD male rats were selected to establish the experimental gastric ulcer model with acetic acid.They were randomly divided into a control group,a model group,a lansoprazole group,a Qingre Jiedu Formula group and a combined drug group.The control group and model group were given distilled water;the lansoprazole group was given lansoprazole;the Qingre Jiedu Formula group was gavaged with Qingre Jiedu Formula;and combined drug group was given lansoprazole and Qingre Jiedu Formula.Gastric ulcer index,inhibition rate of gastric ulcer,pH value of gastric juice,pepsinase activity,pathological changes of gastric tissues and serum levels of epidermal growth factor(EGF)and epidermal growth factor receptor(EGFR)were compared among the groups.Results The gastric ulcer index and pepsinase activity in the Qingre Jiedu Formula group and the lansoprazole group were significantly higher than those in the combined drug group(P<0.05).The inhibition rate of gastric ulcer,gastric juice pH value,serum EGF and EGFR levels in the Qingre Jiedu Formula group and the lansoprazole group were significantly lower than those in the combined drug group(P<0.05).There were no significant differences in gastric ulcer index,pepsin activity,gastric ulcer inhibition rate,gastric juice pH value,serum EGF and EGFR levels between the Qingre Jiedu Formula group and the lansoprazole group(P>0.05).In the lansoprazole group and the Qingre Jiedu Formula group,the degree of gastric mucosal injury was significantly reduced,and the gastric mucosal epithelial degeneration and necrosis and ulcer symptoms were less than in the model group,and gastric mucosal edema,hemorrhage,inflammation and other symptoms were mild and moderate.Gastric mucosal injury was significantly improved in the combined drug group,and the pathological changes of gastric tissue in t
