摘　　要：目的探讨脂蛋白相关磷脂酶A2(Lp-PLA2)水平与心房颤动(房颤)的相关性及其预测价值。方法连续纳入2016~2017年在宜兴市人民医院住院并初次诊断为房颤的31例病人为房颤组,选取同时期入院的73例非房颤病人作为对照组。于入院后第2天检测并比较2组病人的血脂、肝肾功能等生化指标及Lp-PLA2、hs-CRP等炎症指标水平。结果2组年龄、脑梗死史、血清肌酐水平差异有统计学意义(P<0.05)。房颤组Lp-PLA2、hs-CRP水平均明显高于对照组,差异有统计学意义(P<0.01)。Logistic回归分析结果显示,Lp-PLA2是房颤的独立危险因素(OR=12.371,95%CI:1.898~80.648,P<0.05)。ROC分析结果显示,Lp-PLA2预测房颤的AUC为0.821(95%CI:0.727~0.914,P<0.05)。结论Lp-PLA2水平与房颤相关,是预测房颤的独立危险因素。Objective To explore the correlation between lipoprotein-associated phospholipase A2(Lp-PLA2)and atrial fibrillation(AF),and to discuss the predictive value of Lp-PLA2 in AF.Methods From 2016 to 2017,31 elderly patients initially diagnosed with AF were enrolled in this study as AF group,while 73 patients without AF at the same period were selected as the control group.The levels of biochemical indexes including blood lipid,renal function,liver function and inflammatory indexes including high sensitive C reactive protein(hs-CRP)and Lp-PLA2 on the second day after admission were measured and compared between the two groups.Results There were significant differences in age,history of cerebral infarction and serum level of creatinine between the two groups(P<0.05).The levels of Lp-PLA2 and hs-CRP in AF group were significantly higher than those in control group(P<0.01).Logistic regression analysis revealed that Lp-PLA2 was the independent risk factor for AF(OR=12.371,95%CI:1.898-80.648,P<0.05).The area under receiver operating characteristic curve of Lp-PLA2 was 0.821(95%CI:0.727-0.914,P<0.05).Conclusions Lp-PLA2 level is associated with AF,and the high level of Lp-PLA2 is the independent risk factor for AF.
