第2代冷冻球囊消融右下肺静脉致膈神经损伤2例及文献复习

摘　　要：冷冻球囊从第1代发展到第2代,虽然在冷冻性能上有较大的改善,但并没有降低膈神经损伤的发生率,尤其在消融右下肺静脉时,膈神经损伤的发生率还有所增加。中南大学湘雅二医院心血管内科使用第2代冷冻球囊消融阵发性心房颤动,出现了2例膈神经损伤,均发生在消融右下肺静脉时。回顾相关文献,分析本组2例膈神经损伤的可能原因,以期为减少该并发症的发生提供一定的经验。The performance of the second-generation cryoballoon has been greatly improved compared to the first-generation. But it has not reduced the incidence of phrenic nerve injury, and even increased the incidence of this complication during ablation of the right inferior pulmonary vein. Two cases of phrenic nerve injury occurred when right inferior pulmonary vein was ablated with the second-generation cryoballoon in Department of Cardiovascular Medicine, Second Xiangya Hospital, Central South University. Based on literature review, the possible causes for phrenic nerve injury in our report were summarized to provide helpful information for operators aiming to reduce the occurrence of this complication related to the second-generation cryoballoon.
