摘　　要：目的探讨家庭聚集性新型冠状病毒肺炎(COVID-19)患者的CT特点、临床应用价值,以及与核酸检测结果之间的关系。方法回顾性分析3组家庭聚集性COVID-1912例患者的胸部CT表现及核酸检测结果,核酸检测均使用咽拭子方法,比较CT特点与患者的流行病学特点、临床症状以及核酸检测结果之间的关系。结果12例患者中11例首次检查胸部CT有阳性发现,1例患者首次检查CT无阳性征象。3组家庭内各有2例患者连续2次核酸检测为阴性,胸部均出现病毒性肺炎CT征象。其中1组家庭内1例儿童患者,核酸检测阳性,胸部CT无异常改变。3组家庭中其余5例患者核酸检测阳性,胸部CT均出现病毒性肺炎特征。结论对于家庭聚集性COVID-19疑似患者,核酸检测阴性,应进行多次核酸检测并结合CT检查,核酸检测阳性早期未出现影像学异常表现患者应该短期内复查CT。Objective To explore the CT characteristics,clinical application value,and relationship with the results of nucleic acid test in patients with family cluster COVID-19.Methods Chest CT findings and the nucleic acid test results of 12 patients with family cluster COVID-19 were analyzed retrospectively.The nucleic acid test was performed using throat swabs,and the relationship between CT characteristics and the epidemiological characteristics,clinical symptoms and nucleic acid test results of the patients were compared.Results 11 of the 12 patients had positive chest CT findings on the first CT,and 1 patient had no positive CT findings on the first CT.2 patients had consecutive no positive results of nucleic acid test in each of the three families,but CT signs were found in the chest.1 child had positive nucleic acid test but no abnormal changes in chest CT.The rest 5 patients in the 3 families tested positive for nucleic acid,and the chest CT showed the characteristics of viral pneumonia.Conclusion For suspected family cluster COVID-19 patients with negative nucleic acid test,multiple nucleic acid tests should be performed.For patients with positive nucleic acid test and no abnormal imaging manifestations at the early stage,follow-up CT is needed within a short period.
