摘 要:目的探讨不同多巴胺D2受体(DRD2)TaqIA基因型海洛因依赖者默认网络(DMN)差异。方法46例美沙酮维持治疗的男性海洛因依赖者[A1等位基因携带者(A1+组)26例,A2纯合子基因携带者(A1-组)20例]及23例健康对照组(HC组)进行rs-fMRI扫描。3组间匹配年龄、受教育程度及吸烟情况,A1+与A1-组匹配海洛因及美沙酮使用情况。采用单因素方差分析(ANOVA分析)比较3组大脑静息态DMN功能连接差异。分别在A1+组、A1-组及所有海洛因依赖者进行功能连接差异区信号强度与海洛因和美沙酮使用时间、剂量的相关性分析。结果与A1-组相比,A1+组在双侧背内侧前额叶、双侧前扣带、右侧海马/海马旁回/梭状回功能连接减低,在左侧顶下小叶连接增强。在A1+组、A1-组及所有海洛因依赖者海洛因及美沙酮使用时间、剂量与以上脑区功能连接强度值未见明显相关性(P>0.05)。结论不同DRD2 TaqIA基因型海洛因依赖者DMN存在差异,A1+组认知控制、学习记忆相关脑区功能连接减弱,视觉空间注意力相关脑区功能连接增强。Objective To investigate the default mode network(DMN)differences of heroin addicts with different dopamine D2 receptor(I3RD2)TaqIA genes.Methods rs-fMRI scanning was performed on 46 male heroin-dependent individuals[26 heroin addicts with Al allele of the TaqIA(Al+),20 A2 allele carriers(Al-)]under methadone maintenance treatment and 23 healthy control(HC)subjects.Age,education and smoking were matched among the three groups,and heroin and methadone use were matched between Al+and Al-groups.Independent component analysis was used to identify DMN connection and compare differences among groups using A NO VIA.Correlation analysis was conducted between the signal strength of the functional con nectivity difference area,heroin use and methadone treatment history in the A1+group,the Al-group and all heroin addicts,respectively.Results Compared with group Al-group,the A1+group had decreased connections in the bilateral dorsal medial preirontal cortex,bilateral anterior cingulate region,right hippocampus/parahippocampal gyrus/iusiiorm gyrus,and increased connections in the left inferior parietal lobule.No significant correlation was found between the heroin and methadone use and the functional connection strength of the above brain areas in A1+group,Al-group and all heroin addicts(P>0.05).Conclusion There are differences between Al 4 group and Al-group in functional connectivity of the DMN.In the A1+group,functional connections of brain areas related to cognitive control,learning and memory are weakened,while those related to visual spatial attention are enhanced.
