摘  要:目的探讨基于多模型的迭代重建算法(ASIR-V)和自适应统计迭代重建算法(ASIR)在常规剂量胸部CT检查中降低图像噪声和提高图像质量的临床价值。方法选取行宝石能谱CT常规剂量胸部平扫患者30例。所有患者均采用自动管电流调制技术,预设噪声指数为14,对同一患者所得图像采用10%~100%(间隔10%)的不同权重ASIR-V以及40%ASIR进行重建。于气管隆突层面测量各重建图像皮下脂肪,竖脊肌和降主动脉的CT值及标准差SD值,同时计算图像的SNR。2名有经验的放射科医师采用5分法对图像质量进行主观评分。结果所有ASIR-V图像中,皮下脂肪,竖脊肌及降主动脉的SD值随ASIR-V权重的增加逐渐降低,而SNR相应增加,其中,20%ASIR-V^100%ASIR-V图像的噪声明显低于40%ASIR图像,差异有统计学意义(P<0.05)。图像质量主观评价方面,所有ASIR-V图像均能满足临床诊断要求,50%ASIR-V^70%ASIR-V图像质量明显优于40%ASIR及其他ASIR-V图像,并且60%ASIR-V图像对于病灶细微结构的显示最佳。结论在常规剂量胸部CT检查中,相比ASIR算法,ASIR-V技术能够显著降低图像噪声,并且采用60%ASIR-V算法重建的胸部CT图像质量最佳。Objective To explore the clinic value of image quality improvement and noise reduction in routine dose chest CT images by using adaptive statistical iterative reconstruction V(ASIR-V)and adaptive statistical iterative reconstruction(ASIR).Methods 30 patients who underwent routine dose chest CT by using Discovery CT 750 HD were included.All patients adopted automatic tube current modulation technique to obtain a fixed noise index of 14 for the chest CT scan.All the images of the same patient were reconstructed with different percentages of ASIR-V from 10%to 100%in a 10%increment and 40%ASIR,respectively.The CT value and standard deviation(SD)of the subcutaneous fat,erector spinae,and descending aorta were measured at the level of tracheal carina in all reconstructed images,the SNR was also calculated.The subjective image quality was independently evaluated by two experienced radiologists.Results For all ASIR-V images,lhe SD value of fat,erector spinae and descending aorta decreased and SNR increased along with increasing ASIR V.The image noise on 20%ASIR-V to 100%ASIR-V were significantly lower in comparison with 40%ASIR(P<0.05).In terms of subjective image evaluation,all ASIR-V images could satisfy the demands of clinical diagnosis.The image quality of the 50%ASIR-V to 70%ASIR-V series were superior to that of the 40%ASIR and ASIR-V of other percentages,and 60%ASIR-V was the best series of all ASIR-V images,with a highest visibility of small structures.Conclusion In routine dose chest CT,ASIR-V can significantly reduce image noise when compared to ASIR.The chest CT images reconstructed with 60%ASIR-V have the highest image quality.
