当归拈痛汤联合西乐葆治疗急性痛风性关节炎的临床疗效观察

摘　　要：【目的】观察当归拈痛汤联合西乐葆治疗急性痛风性关节炎的临床疗效。【方法】将130例急性痛风性关节炎(湿热蕴结证)患者随机分成治疗组和对照组,每组各65例。对照组给予西乐葆及碳酸氢钠片口服治疗,治疗组在对照组基础上加用当归拈痛汤治疗,治疗周期为1周。观察2组患者治疗前后关节肿胀和疼痛评分以及血尿酸(BUA)、C-反应蛋白(CRP)及血沉(ESR)水平的变化情况。【结果】(1)治疗后,2组患者的关节疼痛及关节肿胀评分均较治疗前明显降低(P<0.05),且治疗组的降低作用均明显优于对照组(P<0.05)。(2)治疗后,2组患者的BUA、ESR、CRP水平均较治疗前明显降低(P<0.05),且治疗组的降低作用均明显优于对照组,差异均有统计学意义(P<0.05)。(3)不良反应情况:治疗结束后,治疗组和对照组各有2例患者出现恶心呕吐等胃肠道反应,予以护胃/止呕等对症处理后恢复正常。【结论】当归拈痛汤联合西乐葆治疗急性痛风性关节炎具有较好的临床疗效,可有效降低血尿酸水平,改善临床症状,控制关节炎症反应。Objective To investigate the efficacy of Danggui Niantong Decoction combined with celebrex capsules for the treatment of acute gouty arthritis. Methods A total of 130 patients with acute gouty arthritis differentiated as damp-heat accumulation syndrome were randomly divided into treatment group and control group,65 cases in each group. Patients in the control group were treated with sodium bicarbonate combined with celebrex capsules orally. Patients in the treatment group were treated with Danggui Niantong Decoction based on the treatment for the control group. The treatment for the two groups lasted one week. Before and after treatment,the changes of the scores of joint swelling and joint tenderness, and the levels of blood uric acid(BUA), C-reactive protein(CRP),erythrocyte sedimentation rate(ESR) in the two groups were observed. Results(1)After one week of treatment, the scores of joint swelling and joint tenderness in the two groups were obviously decreased in comparison with those before treatment(P<0.05),and the decrease in the treatment group was superior to that in the control group(P<0.05).(2)After treatment, the levels of BUA, ESR and CRP in the two groups were obviously decreased in comparison with those before treatment(P<0.05),and the decrease in the treatment group was superior to that in the control group(P<0.05).(3)At the end of trial,there were 2 cases of gastrointestinal reaction such as nausea and vomiting occurred in both of the treatment group and the control group, and the adverse reaction disappeared after symptomatic treatment with stomach-protecting and antiemetic drugs.Conclusion Danggui Niantong Decoction combined with celebrex capsules is effective for the treatment of acute gouty arthritis,and its mechanisms may be associated with the decrease of reducing blood uric acid,relief of clinical symptoms and control of inflammatory reaction in the joints.
