针刺联合育阴丸对卵巢储备功能下降患者月经量及血清VEGF水平的影响研究

摘　　要：【目的】探讨针刺联合育阴丸对卵巢储备功能下降(DOR)患者月经量及血清血管内皮生长因子(VEGF)水平的影响。【方法】将75例西医诊断为DOR,中医诊断为肝肾阴虚型月经过少的患者随机分为3组,其中电针+育阴丸组28例,安慰针+育阴丸组29例,单纯电针组18例,3组患者均给予3个月的相应治疗。观察3组患者治疗前后月经量状况[采用月经失血图(PBAC)评分间接估算经血量(MBL)]及VEGF水平的变化情况。【结果】(1)治疗后,3组患者的月经血量均较治疗前明显改善,MBL积分均较治疗前明显增加(P<0.01)。组间两两比较,电针+育阴丸组患者月经血量的改善程度明显优于其他两组(P<0.05),单纯电针组患者月经血量的改善程度又明显优于安慰针+育阴丸组(P<0.05)。(2)治疗后,3组患者血清VEGF水平均较治疗前明显升高(P<0.01)。组间两两比较,电针+育阴丸组患者血清VEGF水平的改善程度明显优于其他两组(P<0.05),单纯电针组患者血清VEGF升高程度明显优于安慰针+育阴丸组(P<0.05)。(3)安全性评价:治疗过程中,3组患者均无明显不良反应发生,血、尿、大便常规和心电图、肝肾功能检查均无异常变化。【结论】电针联合育阴丸、安慰针联合育阴丸及单纯电针治疗均可改善患者月经情况及细胞因子VEGF水平,其中电针联合育阴丸的疗效最优。Objective To investigate the effect of acupuncture combined with Yuyin Pills on menstrual blood volume and serum cytokine of vascular endothelial growth factor(VEGF)in patients with decreased ovarian reserve function. Methods Seventy-five patients with decreased ovarian reserve function which were differentiated as yin deficiency of liver and kidney,were randomly divided into three groups. The combined group was treated with electroacupuncture and Yuyin Pills(28 cases),Chinese medicine group was given placebo electroacupuncture and Yuyin Pills(29 cases),and acupuncture group was given electroacupuncture alone(18 cases). The three groups were treated for 3 continuous months. Before and after treatment,the menstrual blood volume evaluated indirectly by menstrual blood loss(MBL)with pictorial blood loss assessment chart(PBAC),and the VEGF level in the three groups were observed. Results(1)After treatment,the menstrual blood volume in the three groups was much improved,and MBL scores were markedly increased in comparison with those before treatment(P<0.01). The inter-group comparison showed that the improvement of the menstrual blood volume in the combined group was superior to that in the other two groups(P<0.05),and the improvement of the menstrual blood volume in the acupuncture group was superior to that in the Chinese medicine group(P<0.05).(2)After treatment,serum VEGF level in the three groups was obviously increased in comparison with that before treatment(P<0.01).The inter-group comparison showed that the improvement of the serum VEGF level in the combined group was superior to that in the other two groups(P<0.05), and the improvement of the serum VEGF level in the acupuncture group was superior to that in the Chinese medicine group(P<0.05).(3)During the trial,no obvious adverse reaction occurred in the three groups,or no abnormality was shown in the results of routine test for blood,urine,stool,electrocardiography,liver function,or renal function.
