摘　　要：[目的]总结名中医程锦国治疗肾功能衰竭终末期的临证经验。[方法]通过临床跟师、收集病例,对程锦国主任中医师运用温肾解毒汤治疗终末期肾功能衰竭的理论及思路进行分析总结,并列举医案验证。[结果]名中医程锦国认为",肾劳"体现肾功能衰竭终末期患者以脾失健运、肾失开阖为本,以阴浊内聚、瘀血内停为标的病理特点,以苏叶黄连汤清热和胃、大黄附子汤温阳散寒和四君子汤健脾安中,三方合成温肾解毒汤化裁治之,能够延缓患者病情进展。所举验案为无法接受血液净化治疗的肾功能衰竭终末期患者,辨证为脾肾阳虚、浊毒内蕴,治以温肾健脾、泻浊排毒,方用温肾解毒汤化裁,延缓了病情进展,取得较好疗效。[结论]名中医程锦国运用温肾解毒汤治疗终末期肾功能衰竭,契合病机,疗效确切,值得学习和总结。[Objective]To summarize the clinical experience of famous Doctor of TCM CHENG Jinguo in treating end-stage of renal failure.[Methods]Through following Doctor CHENG’s clinical work and collecting cases,the clinical experience of Doctor CHENG in treating end-stage of renal failure with Wenshen Jiedu Decoction was analyzed,and one medical case was presented.[Results]Doctor CHENG thinks that"consumptive disease of kidney"reflects the pathogenesis characteristics of end-stage of renal failure,which are based on the dysfunction of spleen and kidney,and internal gathering of Yin turbidity and stagnation of blood stasis.Suye Huanglian Decoction focuses on clearing away heat and harmonizing the stomach,Dahuang Fuzi Decoction focuses on warming Yang to expel coldness,and Sijunzi Decoction focuses on tonifying spleen,and the combination of three formulas is Wenshen Jiedu Decoction which can delay the progress of the disease.The patient who was unable to receive blood purification was differentiated as Yang deficiency of spleen and kidney,and accumulation of turbidity and toxin.The patient achieved good results,and the progress of the disease was delayed after treated with Wenshen Jiedu Decoction according to the principles of warming the kidney,strengthening the spleen,reducing turbidity and expelling toxin.[Conclusion]It is suitable with pathogenesis to use Wenshen Jiedu Decoction in treating end-stage of renal failure and has achieved definite efficacy.Doctor CHENG Jinguo’experience in treating end-stage of renal failure is well worth learning and summarizing.
