摘 要:目的:探讨高通量和低通量透析对尿毒症患者钙磷水平的影响。方法:随机数表法将维持性透析治疗的94例患者分为研究组和对照组,每组各47例,分别给予高通量或低通量透析治疗,对比两组患者的临床疗效和血液钙磷水平。结果:单次透析后两组患者血钙、尿素氮、血清肌酐下降率比较,差异无统计学意义(P>0.05);对照组患者血磷水平下降比研究组显著(P<0.05);研究组患者血清全段甲状旁腺激素(iPTH)和β2-微球蛋白(β2-MG)下降率明显高于对照组(P<0.05),对照组患者这两项指标水平下降不明显(P>0.05)。治疗16周后两组患者血钙、尿素氮、血清肌酐水平变化比较,差异均无统计学意义(P>0.05);研究组血磷、iPTH和β2-MG水平与入组前和对照组比较,差异均有统计学意义(P<0.05)。结论:高通量维持透析相较于低通量透析来说,能够增加磷的去除,稳定血钙水平,降低血清iPTH水平,改善机体内钙磷代谢。Objective:To investigate the effects of high-throughput and low-flux dialysis on calcium and phosphorus levels in uremic patients.Methods:A total of 94 patients undergoing maintenance dialysis treatment were enrolled in the study.According to the random number table method,47 patients in the study group and the control group were given.Flux dialysis treatment,the control group was given low-flux dialysis treatment,the clinical efficacy and calcium and phosphorus levels of the two groups were compared.Results:There was no significant difference in blood calcium,urea nitrogen and serum creatinine between the two groups after single dialysis(P>0.05),but the serum phosphorus level decreased significantly(P<0.05).The decrease rate of iPTH andβ2-MG of in the study group was significantly higher than that of the control group(P<0.05).The level of these two indexes was not significantly decreased in the control group(P>0.05).There was no difference in the changes of serum calcium,urea nitrogen and serum creatinine levels between the two groups before admission and 16 weeks after treatment(P>0.05).The levels of serum phosphorus,iPTH andβ2-MG in the study group were significantly higher than those before and in the control group,the differences were statistically significant(P<0.05).Conclusion:High-throughput maintenance dialysis can increase phosphorus clearance,stabilize blood calcium levels,lower serum PTH levels,and improve calcium and phosphorus metabolism in the body compared with low-flux dialysis.
