摘 要:目的利用动态增强MR肾图量化观察兔单侧输尿管梗阻(UUO)及UUO再通(RUUO)术后对侧肾功能改变情况。方法将50只新西兰大白兔随机分5组,每组10只。其中对照组进行UUO假手术,其余4组手术建立右侧输尿管梗阻再通模型,分别于梗阻后第3、7、14、28 d行RUUO术,均于再通术前以及术后1个月行动态增强MR并绘制MR肾图,根据Baumann-Rudin模型计算肾小球滤过系数(kcl)。对5组kcl数据进行单因素方差分析,组间两两比较采用LSD-t检验,RUUO术前、术后比较采用配对样本t检验。结果UUO术后3、7、14、28 d组的kcl均高于对照组(P<0.05),但4组间比较差异无统计学意义(P>0.05)。RUUO术后1个月28 d组kcl低于其他3个模型组(P<0.05),但与对照组间差异无统计学意义(P>0.05);7 d组的kcl最高,但仅与对照组和28 d组比较差异有统计学意义(P<0.05),其他各组间差异无统计学意义(P>0.05)。对照组梗阻及再通后kcl差异无统计学意义(P>0.05);3、7、14、28 d组梗阻后kcl均高于RUUO术后1个月(P<0.05)。结论动态增强MR肾图能够量化肾功能改变,梗阻时间过长会造成对侧肾功能的不可逆损伤。Objective Contralateral renal function changes after unilateral ureteral obstruction(UUO)and UUO recanalization(RUUO)were observed quantitatively with dynamic enhanced MR renal renography.Methods All of 50 New Zealand white rabbits were randomly divided into 5 groups,10 in each group.The control group underwent sham UUO operation and the other four groups were operated to establish right ureteral recanalization obstruction model.The recanalization was performed on the 3rd,7th,14th and 28th day after the obstruction.All rabbits underwent dynamic enhanced MR before and 1 month after the recanalization,and the magnetic resonance renography was drawn at the same time.The glomerular filtration coefficient(kcl)was calculated based on the Baumann-Rudin model.One-way ANOVA with LSD post-hoc test was used to compare the kcl among the five groups,and paired-samples t test was used for comparison between before and after RUUO operation.Results The kcl of 3 d,7 d,14 d and 28 d groups after UUO were higher than that of the control group(P<0.05),but there was no significant difference among the four groups(P>0.05).One month after RUUO,the kcl of the 28 d group was significantly lower than that of the other three model groups(P<0.05),but there was no significant difference compared with the control group(P>0.05).The kcl of the 7 d group was the highest,but the difference was statistically significant(P<0.05)only when compared with the control group and the 28 d group,and there was no significant difference between the other groups(P>0.05).There was no significant difference between the kcl after UUO and RUUO in the control group(P>0.05).The kcl of 3 d,7 d,14 d and 28 d groups after UUO were significantly higher than that of 1 month after RUUO(P<0.05).Conclusion Dynamic enhanced MR renography can quantify the changesinrenal function.Long obstruction time can cause irreversible damage to the contralateral renal function.
