摘　　要：目的探索以奥马哈系统理论为指导对糖尿病患者实施延续性护理的效果与应用价值。方法选择昆明医科大学附属延安医院呈贡医院糖尿病患者200例,随机分为两组,对照组给予常规护理,观察组在常规护理的基础上,引入奥马哈系统理论标准化护理语言,开展延续性护理。根据病情给予出院时评估和家庭回访,动态观察记录患者健康状况,及时反馈发现的问题,实施护理干预,提高患者自我管理能力和生活能力。结果观察组患者的生活质量评分,干预后成效评分显著高于对照组。干预后空腹血糖、餐后2 h血糖、HbA1C、SMBG明显低于对照组(P<0.05),差异有统计学意义。结论奥马哈系统理论对糖尿病患者的自我管理能力、日常生活能力、精神状况等具有明显的改善,能有效提高患者的自我护理能力及对疾病的认知,一定程度上延缓病情发展,从而降低了并发症发生。Objective To explore the effect and application value of continuous nursing care for patients with diabetes under the Omaha System.Methods A total of 200 diabetic patients were randomly divided into 2 groups.The control group recived routine nursing while the observation group received continuous nursing with the combination of routine nursing and standardized nursing language that is based on Omaha System.Evaluation at the time of discharge and family visit were conducted to observe patients’health condition,with the purpose of enhancing their self-management and living ability.Results Quality of life scale in observation group was significantly higher than that in the control group.FBS,2 hPBG,HbA1C and SMBG in the observation group were significantly lower than those in the control group(P<0.05).Conclusions With the conduction of Omaha System theory in continuous nursing,self-management ability,living ability and mental conditions of diabetic patients can be improved significantly and their awareness of illness and self-nursing can be raised,which will slow the progression of the disease and reduce complications.
