隔药灸治疗寒凝血瘀证原发性痛经患者焦虑和抑郁的疗效观察

摘　　要：【目的】观察隔药灸对寒凝血瘀证原发性痛经患者治疗前后焦虑和抑郁的影响。【方法】本研究分为回顾性研究和前瞻性研究两部分。收集100例非原发性痛经和100例原发性痛经患者的自评焦虑量表(self-rating anxiety scale,SAS)、自评抑郁量表(self-rating depression scale,SDS)资料,进行回顾性分析。基于回顾性研究结果,招募30例寒凝血瘀证原发性痛经患者,取穴神阙、关元,给予隔药灸治疗,连续治疗3个月经周期。比较患者治疗前后SAS、SDS评分和痛经视觉模拟评分(dysmenorrhea visual analogue score,VAS)的变化情况。【结果】(1)原发性痛经组与非原发性痛经组在月经期间测试SAS、SDS评分显示,原发性痛经组患者SAS和SDS评分均明显高于非原发性痛经组,差异均有统计学意义(P<0.05)。(2)经隔药灸治疗后,30例寒凝血瘀证原发性痛经组患者的SAS、SDS以及VAS评分均明显改善,与治疗前比较,差异均有统计学意义(P<0.05或P<0.01)。经隔药灸治疗后,寒凝血瘀证原发性痛经组患者的SAS、SDS以及VAS评分均明显改善,与非原发性痛经组比较,差异均有统计学意义(P<0.05)。【结论】隔药灸能改善寒凝血瘀证原发性痛经患者焦虑、抑郁的状态。Objective To observe the effects of herb partition moxibustion on anxiety and depression of patients with primary dysmenorrhea of cold accumulation causing blood stasis syndrome before and after treatment.Methods There were retrospective study and prospective study in the paper.The data of self-rating anxiety scale(SAS)and self-rating depression scale(SDS)of 100 non-primary dysmenorrhea women and 100 primary dysmenorrhea patients were collected and analyzed retrospectively.Based on the results of the retrospective study,30 cases of patients of primary dysmenorrhea with accumulation causing blood stasis syndrome were recruited,and were given herb partition moxibustion at acupoints of Shenque(CV8)and Guanyuan(CV4)for 3 continous menstrual cycles.The changes of SAS,SDS and dysmenorrhea visual analogue scale(VAS)pain score were observed before and after treatment.Results(1)The results of SAS and SDS scores during dysmenorrhea showed that SAS and SDS scores of primary dysmenorrhea patients were significantly higher than those of non-primary dysmenorrhea women,the difference being statistically significant(P<0.05).(2)After herb partition moxibustion treatment,SAS,SDS and VAS scores of 30 cases of patients with primary dysmenorrhea of cold accumulation causing blood stasis syndrome were improved as compared with those before treatment,the difference being statistically significant(P<0.05 or P<0.01).After herb partition moxibustion treatment,SAS,SDS and VAS scores of the patients with primary dysmenorrhea of cold accumulation causing blood stasis syndrome were improved as compared with those of the patients with non-primary dysmenorrhea,the difference being statistically significant(P<0.05).Conclusion Herb partition moxibustion can improve anxiety and depression of patients with primary dysmenorrhea of cold accumulation causing stasis syndrome.
