摘　　要：目的 评价早期功能锻炼及以健康信念模式为指导的健康教育护理干预对中老年髋、膝关节置换术后下肢深静脉血栓(DVT)的预防作用。方法 选取北京大学深圳医院2017年2月至2018年2月收治的98例接受髋、膝关节置换术治疗患者作为研究对象,按照区组随机原则分为对照组和观察组,每组49例,对照组行术后常规护理,观察组则应用早期功能锻炼及以健康理念模式为指导健康教育模式,评价两组不同护理应用后的效果。结果 观察组护理实施后的疾病相关知识知晓评分、髋膝关节功能评分均高于对照组,差异有统计学意义(均P<0.05),下肢DVT发生率少于对照组,差异有统计学意义(P<0.05)。结论 中老年髋膝关节置换术后应用早期功能锻炼结合以健康信念模式为指导的健康教育,可有效预防术后下肢DVT发生。Objective To evaluate the preventive effects of early functional exercise and health education nursing intervention guided by health belief model on deep vein thrombosis(DVT)in middle-aged and elderly patients with hip and knee joint replacement.Methods A total of 98 patients who underwent hip and knee arthroplasty were included in the study.They were divided into two groups according to the randomized principle of the block,with 49 patients in each group.The control group only received routine postoperative care,while the observation group applied early functional exercise and health education model as the guiding health education model.The effects of two groups of different nursing applications were evaluated.Results The disease-related knowledge awareness scores and hip-knee joint function scores of the observation group were higher than those of the control group,and the differences were statis?tically significant(P<0.05).The incidence of deep venous thrombosis was lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusions Early functional exercise combined with health belief model guided by hip and knee joint replacement in middle-aged and elderly patients can effectively prevent postoperative deep venous thrombosis of lower extremity,and its clinical application value is significant.
