剖宫产产妇寒战发生情况及其影响因素分析

摘　　要：目的探讨剖宫产产妇寒战发生情况及其影响因素。方法选取2015年2月~2018年6月在我院进行剖宫产分娩的产妇84例进行回顾性分析,收集产妇年龄、BMI指数、职业、民族、产次、中心体温、手术时间、术中出血量、椎管内麻醉阻滞方式、麻醉阻滞平面、焦虑、抑郁情况等资料。单因素分析采用卡方检验,多因素分析采用Logistic回归分析。结果84例产妇中51例出现寒战,寒战发生率为60.71%。单因素分析结果显示产妇年龄、BMI指数、中心体温、手术时间、手术出血量、麻醉阻滞平面、焦虑、抑郁情况的不同剖宫产产妇寒战发生率差异均有统计学意义(P<0.05)。多因素分析结果显示:年龄(OR=0.531,95%CI=0.407~0.690),BMI指数(OR=0.503,95%CI=0.429~0.664),中心体温(OR=0.628,95%CI=0.430~0.759),手术出血量(OR=1.408,95%CI=1.217~1.526),麻醉阻滞平面(OR=0.488,95%CI=0.208~0.593),SDS(OR=1.593,95%CI=1.306~1.694)为剖宫产产妇寒战独立性影响因素。结论剖宫产产妇寒战发生率较高,其主要与产妇年龄、BMI指数、中心体温、手术出血量、麻醉阻滞平面及抑郁有关。Objective To explore the occurrence of shivering in puerpera undergoing cesarean section and influencing factors.Methods 84 puerpera who underwent cesarean section in our hospital from February 2015 to June 2018 were selected and retrospectively analyzed.The data of maternal age,BMI index,occupation,nationality,production times,central temperature,operation time,intraoperative bleeding volume,intraspinal anesthesia block mode,anesthesia block plane,anxiety and depression were collected by retrospective analysis.Univariate analysis was performed using chi-square test and multivariate analysis using Logistic regression analysis.Results There were 51 cases of shivering in 84 puerpera,and the incidence of shivering was 60.71%.Univariate analysis showed that age,BMI index,central temperature,operation time,bleeding volume,anesthesia block plane,anxiety and depression of puerpera undergoing cesarean section had statistically significant differences(P<0.05).Multi-factor analysis showed age(OR=0.531,95%CI=0.407-0.690),BMI index(OR=0.503,95%CI=0.429-0.664),central body temperature(OR=0.628,95%CI=0.430-0.759),surgical bleeding volume(OR=1.408,95%CI=1.217-1.526),anesthetic block plane(OR=0.488,95%CI=0.208-0.593)and SDS(OR=1.593,95%CI=1.306-1.694)were independent influencing factors of shivering in puerpera undergoing cesarean section.Conclusion The incidence of shiveringin puerpera undergoing cesarean section is high,which is mainly related to maternal age,BMI index,central body temperature,surgical bleeding volume,anesthesia block plane and depression.
