右美托咪定与瑞芬太尼复合依托咪酯用于宫腔镜手术的麻醉效果对比

摘　　要：目的比较右美托咪定与瑞芬太尼复合依托咪酯用于宫腔镜手术的麻醉效果。方法选取2017年1月~2018年12月我科收治的行宫腔镜手术患者120例,随机数字表法分为对照组(瑞芬太尼复合依托咪酯麻醉,60例)与观察组(右美托咪定复合依托咪酯麻醉,60例),比较两组患者不同时间点(T0、T1、T2、T3、T4)的心率(HR)、平均动脉压(MAP)、呼吸频率(RR)水平;麻醉起效时间、苏醒时间、拔管时间;苏醒期间Ramsay镇静评分、术后10min警觉/镇静评分(OAA/S评分);不良反应情况。结果观察组患者T1、T2、T3、T4不同时间点的HR、MAP、RR均高于对照组(P<0.05),观察组患者麻醉起效时间、苏醒时间、拔管时间均短于对照组(P<0.05);观察组患者Ramsay镇静评分高于对照组,而OAA/S评分低于对照组(P<0.05);观察组患者不良反应发生率低于对照组(P<0.05)。结论针对宫腔镜手术患者,右美托咪定复合依托咪酯用于麻醉过程中,麻醉效果优、镇静作用强,对患者循环、呼吸影响小,且更利于术后苏醒,且更安全。Objective To compare the anesthetic effect of dexmedetomidine and remifentanil combined with etomidate for hysteroscopic surgery.Methods 120 patients who underwent hysteroscopy in our department from January 2017 to December 2018 were randomly divided into the control group(remifentanil and etomidate anesthesia)and the study group(Dexmedetomidine and etomidate anesthesia),with 60 cases in each group.heart rate(HR),mean arterial pressure(MAP),and respiratory rate(RR)of patients at different time points(T0,T1,T2,T3,T4),anesthesia onset time,wake-up time,extubation time,ramsay sedation score during wake-up,alertness/sedation score(OAA/S score)at 10 mins after operation,adverse reactions were compared between the two groups.Results The HR,MAP,and RR of T1,T2,T3,and T4 at different time points in the study group were higher than those in the control group(P<0.05).The anesthesia onset time,wake-up time,and extubation time in the study group were shorter than those in the control group(P<0.05);The Ramsay sedation score of the study group was higher than that in the control group,while the OAA/S score was lower than that in the control group(P<0.05);The incidence of adverse reactions in the study group was lower than that in the control group(P<0.05).Conclusion Dexmedetomidine combined with etomidate has better anesthesia and sedative effect in the treatment of patients undergoing hysteroscopy surgery.It also has slight influence on the circulation and breathing of patients,and is more conducive to postoperative recovery and has good safety.
