运用PDCA提高护士对恶性心律失常早期识别中的应用效果

摘　　要：目的探讨PDCA循环法在提高护士对恶性心律失常早期识别中的应用效果。方法选取2018年1月~2019年3月肇庆市第一人民医院心内科护士20名作为研究对象,将2018年1~8月收治106例患者作为培训前观察对象,2018年9月~2019年3月89例患者作为培训后观察对象。对本科20名护士,进行提高恶性心律失常识别率培训。邀请医生给参加培训护士统一进行PPT讲课,微视频授课、结合案例情景模拟演练、典型个案,以小组、工作坊讨论形式讨论。制定专科疾病护理常规及专科疾病应急流程,编写恶性心律失常典型心电图表现的图本,强化护士的恶性心律失常知识。比较培训前后护士对恶性心律失常理论知识与心电图识别能力。结果运用PDCA循环法提高护士的理论成绩与操作成绩,培训后护士对恶性心律失常的理论成绩(t=5.322)与操作成绩(t=4.353)培训前后对比差异有统计学意义(P<0.01),护士对恶性心律失常的识别率,由培训前为50%上升至80%,差异有统计学意义(χ^2=14.581,P<0.05),患者对护理工作的满意度比较,培训前后差异无统计学意义(P>0.05),但患者对护理满意度有升高趋势,由84.90%提高到91.01%。结论运用PDCA循环法能提高心血管护士对恶性心律失常早期识别率,培养护士对专科疑难问题的处理能力,抢救能力和保证培训的质量。Objective To explore application effect of PDCA on improving early identification of nurses in malignant arrhythmia.Methods 20 nurses in department of cardiology in Zhaoqing First People's Hospital from January 2018 to March 2019 were selected as research objects.106 patients who were admitted and treated from January to August 2018 were set as the observation objects before training.89 patients who were admitted and treated from September 2018 to March 2019 were selected as observation objects after training.Twenty undergraduate nurses were trained to improve the recognition rate of malignant arrhythmia.Doctors were invited to give nurses training by unified PPT lectures,micro-video lectures,combined case scenario simulation exercises,typical cases,discussion in the form of group and workshop discussions.The nursing routine and emergency procedures of specialized diseases were developed,the picture book of typical ECG manifestations of malignant arrhythmia was complied and the knowledge of malignant arrhythmia of nurses was strengthened.The theoretical knowledge of malignant arrhythmia and ECG recognition ability of nurses before and after training were compared.Results PDCA cycle method was used to improve the theoretical achievement and operational scores of nurses.Statistical study data showed that the theoretical scores of nurses on malignant arrhythmias(t=5.322)and operation scores(t=4.353)had statistically significant differences before and after training(P<0.01).The rate of recognition of malignant arrhythmia by nurses increased from 50% before training to 80%.The difference was statistically significant(χ^2=14.581,P<0.05).There was no statistically significant difference in the satisfaction of patients with nursing work before and after training(P>0.05).However,the patient's satisfaction with nursing has an increasing trend,with an increase from 84.90%to 91.01%.Conclusion The application of PDCA cycle method can improve the rate of early recognition of malignant arrhythmia,train nurses'ability to deal w
