腹腔镜下治疗中高位隐睾150例临床分析

摘　　要：目的分析中高位隐睾患儿应用腹腔镜微创治疗的临床手术效果。方法收集2016年1月~2017年12月应用腹腔镜治疗中高位隐睾150例,全部为一期下降固定术。结果142例患儿在腹腔镜下完成手术,8例中转开放,中位132例,高位18例,单侧隐睾112例,双侧隐睾38例,其中睾丸发育一般7例,先天无睾丸1例,合并对侧内环口未闭35例,1例行睾丸切除,手术过程顺利,术后150例得到随访,140例固定于阴囊内,睾丸无萎缩,血运良好,10例固定至外环口或阴囊上方处,血运可,无睾丸萎缩;对比中位隐睾组及高位隐睾组近期并发症,高位隐睾组高于中位隐睾组,差异有统计学意义(P<0.05);两组远期并发症比较,差异无统计学意义(P>0.05)。结论经腹腔镜下治疗患儿中高位隐睾具有辅助诊断、微创、操作简单、临床疗效好、美观等优点,尤其是对中位、双侧型隐睾更具优势。Objective To analyze the clinical surgical effect of laparoscopic minimally invasive treatment in middle and high cryptorchidism in children.Methods 150 patients with middle and high cryptorchidism who were treated by laparoscopy from January 2016 to December 2017 were selected,and they were treated by one-stage descending fixation.Results 142 children completed the operation under laparoscopy,and 8 patients were converted to open.There were 132 cases of middle cryptorchidism and 18 cases of high cryptorchidism.There were 112 cases of unilateral cryptorchidism and 38 cases of bilateral cryptorchidism.Among them,there were 7 cases with normal testicular development,1 case with congenital absence of testicle and 35 cases with contralateral internal ring orifice closure.1 case underwent orchiectomy and the operation went smoothly.150 patients were followed up after surgery.In 140 cases,the testicles were fixed in scrotum without atrophy and had good blood supply.In 10 cases,the testis was fixed to the outer ring or above the scrotum with good blood supply and there was no testicular atrophy.Recent complications of the high cryptorchidism group were higher than the middle cryptorchidism group,and the difference was statistically significant(P<0.05).There were no significant differences in long-term complications between the two groups(P>0.05).Conclusion Laparoscopy in treatment of children with middle and high cryptorchidism has the advantages of auxiliary diagnosis,minimally invasive,simple operation,good clinical curative effects and beautiful appearance,especially for the middle and bilateral cryptorchidism.
