摘  要:目的:分析人胰高糖素样肽1类似物利拉鲁肽联合骨髓间充质干细胞移植对急性肺损伤的保护作用。方法:将50只BALB/c小鼠随机分5组,每组10只:正常组尾静脉注射1 ml生理盐水;其余4组均建立急性肺损伤模型,细胞组尾静脉注射1 ml同种异体骨髓间充质干细胞悬液;药物组腹腔注射100μg/kg利拉鲁肽;实验组尾静脉注射1 mL同种异体骨髓间充质干细胞悬液,腹腔注射100μg/kg利拉鲁肽。造模后24 h,获取肺泡灌洗液与双侧肺组织,分别进行炎性细胞因子、肺组织细胞凋亡、肺组织湿干质量比、肺组织病理及肺组织甲状腺转录因子1基因与蛋白检测。结果:同正常组对比,模型组肿瘤坏死因子α、白细胞介素1β、白细胞介素6浓度显著升高(P<0.05);细胞组、药物组、实验组肿瘤坏死因子α、白细胞介素1β、白细胞介素6浓度均较模型组显著降低(P<0.05),并且实验组上述炎性因子浓度低于细胞组、药物组(P<0.05);正常组凋亡细胞最少,模型组可见大量凋亡细胞,药物组与细胞组凋亡细胞数较模型组明显减少,实验组凋亡细胞较细胞组、药物组进一步减少;模型组肺组织湿干质量比高于正常组(P<0.05),细胞组、药物组、实验组肺组织湿干质量比低于模型组(P<0.05),实验组低于细胞组、药物组(P<0.05);HE染色显示,模型组肺泡塌陷出血,肺泡间隔增厚,大量炎性细胞浸润;细胞组、药物组肺部结构破坏程度相较模型组明显减轻,两组差异不明显;实验组肺泡结构明显改善,接近正常组;模型组甲状腺转录因子1基因与蛋白表达低于正常组(P<0.05),细胞组、药物组、实验组甲状腺转录因子1基因与蛋白表达高于模型组(P<0.05),实验组甲状腺转录因子1基因与蛋白表达高于细胞组与药物组(P<0.05);结论:结果提示利拉鲁肽联合骨髓间充质干细胞移植治疗急性肺损伤的效果优于单一治疗方式,并且其肺保护作用可能与甲Objective:To analyze the protective effect of human glucagon-like peptide 1 analogue liraglutide combined with bone marrow mesenchymal stem cell transplantation on acute lung injury.Methods:Fifty BALB/c mice were randomly divided into five groups with 10 mice in each group:the normal group did not receive any treatment;the other four groups established acute lung injury models.In the cell group,1 ml allogeneic bone marrow mesenchymal stem cell suspension was injected into the tail vein;in the drug group,100μg/kg liraglutide was injected into the abdominal cavity;in the experimental group,1 ml allogeneic bone marrow mesenchymal stem cell suspension was injected into the tail vein,and 100μg/kg liraglutide was injected into the abdominal cavity.24 hours after the establishment of the model,alveolar lavage fluid and bilateral lung tissues were obtained.Inflammatory cytokines,apoptosis of lung tissues,wet-dry mass ratio of lung tissues,pathology of lung tissues and thyroid transcription factor-1 gene and protein in lung tissues were detected respectively.Results:Compared with the normal group,the concentration of TNF-α,IL-1beta and IL-6 in the model group increased significantly(P<0.05);the concentration of TNF-α,IL-1beta and IL-6 in the cell group,drug group and experimental group decreased significantly(P<0.05),and the concentration of the above inflammatory factors in the experimental group was lower than that in the cell group and drug group(P<0.05).The apoptotic cells in the normal group were the least,and a large number of apoptotic cells were observed in the model group.The number of apoptotic cells in the drug group and the cell group was significantly lower than that in the model group.The apoptotic cells in the experimental group were further reduced than those in the cell group and the drug group.The wet-dry mass ratio of lung tissue in model group was higher than that in normal group(P<0.05).The wet-dry mass ratio of lung tissue in cell group,drug group and experimental group was lower than that in mod
