摘  要:目的:探讨细胞DNA定量分析技术(DNA-ICM)在胸腹腔积液细胞诊断中的临床价值。方法:选取本院2016年10月-2018年12月胸腹腔积液标本366例,以病理诊断结果为金标准,比较DNA-ICM、脱落细胞学(LBC)及联合检测诊断恶性胸腹腔积液的敏感度、特异度、阳性预测值和阴性预测值。结果:DNA-ICM与联合检测的诊断符合率均高于LBC(P<0.05)。DNA-ICM与联合检测的漏诊率均低于LBC(P<0.05)。DNA-ICM与联合检测的诊断符合率与漏诊率比较,差异均无统计学意义(P>0.05)。DNA-ICM的敏感度与阴性预测值均高于LBC(P<0.05);联合检测的敏感度、特异度、阳性预测值及阴性预测值均高于LBC(P<0.05)。联合检测的敏感度、特异度、阳性预测值及阴性预测值与DNA-ICM比较,差异均无统计学意义(P>0.05)。结论:DNA-ICM在胸腹腔积液诊断中具有较高的临床应用价值,联合LBC可明显提高诊断准确率、明确胸腹腔积液性质及细胞分化程度。Objective:To investigate quantitative analysis of DNA image cytometry (DNA-ICM) in diagnosis of the pleural effusion and ascites.Method:From October 2016 to December 2018,366 cases of pleural effusion and ascites samples were selected.The results of pathological diagnosis as the gold standard,the sensitivity,specificity,positive predictive value and negative value of DNA-ICM,liquid-base cytology (LBC) and combined detection in the diagnosis of malignant pleural effusion and ascites were compared.Result:The diagnostic coincidence rate of DNA-ICM and combined detection was higher than that of LBC (P<0.05).The missed diagnosis rate of DNA-ICM and combined detection was lower than that of LBC (P<0.05).There were no significantly differences in the diagnostic coincidence rate and missed diagnosis rate between DNA-ICM and combined detection (P>0.05).The sensitivity and negative predictive value of DNA-ICM were higher than those of LBC (P<0.05).The sensitivity,specificity,positive predictive value and negative predictive value of the combined detection were all higher than those of LBC (P<0.05).There were no significantly differences in the sensitivity,specificity,positive predictive value and negative predictive value between the DNA-ICM and combined detection (P>0.05).Conclusion:DNA-ICM has a high clinical application value in the diagnosis of pleural effusion and ascites.Combined with LBC,can significantly improve the diagnostic accuracy,identify the nature of pleural effusion and ascites and the degree of cell differentiation.
