强力定眩胶囊与针刺法联合治疗后循环缺血性眩晕合并高血压患者的效果观察及研究机制

摘　　要：目的:探究强力定眩胶囊联合针刺法治疗后循环缺血性眩晕合并高血压患者的效果及作用机制。方法:选取2018年1月-2019年1月就诊于本院神经外科的82例后循环缺血性眩晕合并高血压患者临床资料进行回顾性分析,根据治疗方法不同分为对照组与观察组,每组41例。对照组进行常规治疗,观察组进行强力定眩胶囊联合针刺法治疗。比较两组治疗前后血浆5-羟色胺(5-HT)、去甲肾上腺素(NE)、γ-氨基丁酸(GABA)、椎-基底动脉平均血流速度、眩晕程度及治疗效果。结果:治疗后,观察组5-HT与GABA均低于对照组,而NE高于对照组(P<0.05);治疗后,观察组眩晕程度评分低于对照组,而椎-基底动脉平均血流速度高于对照组(P<0.05);观察组治疗总有效率为97.56%高于对照组的78.05%(P<0.05)。结论:采用强力定眩胶囊联合针刺法治疗后循环缺血性眩晕合并高血压患者可改善眩晕程度,增加椎-基底动脉平均血流速度,疗效显著,具有一定的推广价值。Objective:To explore the effect and mechanism of Qiangli Dingxuan Capsule combined with acupuncture in the treatment of patients with posterior circulation ischemic vertigo complicated with hypertension.Method:The clinical data of 82 patients with posterior circulation ischemic vertigo complicated with hypertension admitted to the neurosurgery Department of our hospital from January 2018 to January 2019 were selected for retrospectively analyzed.According to different treatment methods,they were divided into control group and observation group,41 cases in each group.The control group was treated with conventional treatment,and the observation group was treated with Qiangli Dingxuan Capsule combined with acupuncture.The plasma serotonin (5-HT),norepinephrine (NE),γ-aminobutyric acid (GABA),vertebra-basilar artery mean blood flow velocity,vertigo degree before and after treatment and therapeutic effect were compared between the two groups.Result:After treatment,5-HT and GABA in the observation group were lower than those in the control group,while NE was higher than that in the control group (P<0.05).After treatment,the vertigo score of the observation group was lower than that of the control group,while the vertebra-basilar artery mean blood flow velocity was higher than that of the control group (P<0.05).The total effective rate in the observation group was 97.56 % higher than 78.05% in the control group (P<0.05).Conclusion:Qiangli Dingxuan Capsule combined with acupuncture in the treatment of patients with posterior circulation ischemic vertigo complicated with hypertension can improve the degree of vertigo and increase the vertebra-basilar artery mean blood flow velocity.It has obvious therapeutic effect and certain promotion value.
