中孕期脐动脉血流频谱检查对单脐动脉胎儿畸形的诊断价值

摘　　要：目的:分析中孕期脐动脉血流频谱检查对单脐动脉胎儿畸形的诊断价值。方法:选取2012年3月-2018年11月本院诊断的60例单脐动脉胎儿,根据是否存在畸形将其分为对照组(健康胎儿,n=34)与观察组(畸形胎儿,n=26),两组均在中孕期开展了彩超脐动脉血流频谱检查,对两组胎儿的血流动力学参数进行比较,从而评估中孕期脐动脉血流频谱检查对单脐动脉胎儿畸形的诊断价值。结果:观察组单脐动脉胎儿的畸形类型为心血管畸形8例(30.77%)、肢体畸形2例(7.69%)、中枢神经畸形5例(19.23%)、泌尿系统畸形10例(38.46%)、其他畸形1例(3.85%)。观察组中孕期脐动脉血流频谱检查表现PI为(1.03±0.21)、S/D为(3.07±0.54)、RI为(0.67±0.09),均明显高于对照组(P<0.05)。同时,观察组胎儿引产19例(73.08%),娩出3例(11.54%),死亡4例(15.38%),其引产率与死亡率均显著高于对照组(P<0.05)。结论:中孕期脐动脉血流频谱检查对单脐动脉胎儿畸形的诊断有指导价值,在中孕期开展该项检查可提高单脐动脉胎儿畸形的诊断准确率,对抑制围生儿死亡具有良好价值。Objective:To analyze the diagnostic value of umbilical artery blood flow spectrum in fetal malformation.Method:A total of 60 single umbilical artery fetuses diagnosed in our hospital from March 2012 to November 2018 were selected and divided into the control group (healthy fetuses,n=34) and observation group (malformed fetuses,n=26) according to whether malformations.Color Doppler ultrasound umbilical artery blood flow spectrum examination was carried out in two groups during the middle pregnancy,and the hemodynamic parameters of two groups were compared statistically in order to evaluate the value of the umbilical artery blood flow spectrum examination during the middle pregnancy in the diagnosis of single umbilical artery fetuses malformation.Result:8 cases (30.77%) were cardiovascular malformation,2 cases (7.69%) were limb malformation,5 cases (19.23%) were central nervous malformation,10 cases (38.46%) were urinary system malformation,1 case (3.85%) was other malformation in the observation group.The frequency spectrum of umbilical artery blood flow in the observation group,PI was (1.03±0.21),S/D was (3.07±0.54),RI was (0.67±0.09),which were significantly higher than those in the control group (P<0.05).Meanwhile,19 cases (73.08%) were induced labor,3 cases (11.54%) were delivered,and 4 cases (15.38%) died in the observation group,the induced labor rate and mortality rate were significantly higher than those in the control group (P<0.05).Conclusion:Umbilical artery blood flow spectrum examination in middle pregnancy has a guiding value in the diagnosis of single umbilical artery fetuses malformation.The examination in middle pregnancy can promote the diagnostic accuracy of single umbilical artery fetal malformation,and has a good value for inhibiting perinatal death.
