基层医院开展儿童早期综合发展指导工作模式的探讨

摘　　要：目的:探讨适合基层医院开展的儿童早期综合发展(integrated early child development,IECD)指导工作模式。方法:选取2017-2018年儿童保健门诊200例儿童作为研究对象,按随机数字表法分为观察组和对照组,每组100例。对照组给予常规系统保健,观察组在对照组基础上给予IECD指导。比较两组在6、12、18月龄时体格发育和神经心理发育情况。结果:观察组12、18个月身长与体重均明显高于对照组(P<0.05);观察组6、12、18个月应物能、精细动作、语言能及应人能的DQ均高于对照组,且观察组6、18个月大动作的DQ均高于对照组(P<0.05);观察组6、12、18个月中上及优秀率均明显高于对照组(P<0.05)。结论:IECD指导对儿童的体格发育和神经心理发育有明显促进作用,早教班和专案门诊相结合的工作模式在基层医院易于开展,值得临床推广应用。Objective:To explore the suitable working mode of integrated early child development (IECD) for primary hospitals.Method:A total of 200 children in the outpatient Department of children’s health care in our hospital from 2017 to 2018 were selected as study objects.They were divided into observation group and control group according to the random number table method,100 cases in each group.The control group was given routine systematic health care,and the observation group was given IECD guidance on the basis of the control group.Physical development and neuropsychological development were compared between the two groups at 6,12 and 18 months of age.Result:The body length and weight of the observation group at 12 and 18 months of age were significantly higher than those of the control group (P<0.05).The DQ for ability to respond to things,fine motor,language and respond to people of the observation group at 6,12 and 18 months of age were higher than those of the control group,and the DQ for big action of the observation group at 6,12 and 18 months of age were higher than those of the control group (P<0.05).The above average and excellent rate of 6,12 and 18 months of age in the observation group were significantly higher than those in the control group (P<0.05).Conclusion:The guidance of IECD can obviously promote children’s physical development and neuropsychological developmet,and the working mode combining early education class and special outpatient service is easy to develop in primary hospitals,which is worthy of clinical application.
