摘 要:目的:观察COM.TEC型血细胞分离机AutoMNC程序采集外周血单个核细胞时ACD与全血不同体积比对供者采血前后血液、离子及安全性的影响.方法:应用COM.TEC型血细胞分离机采集经粒细胞刺激因子动员的供者外周血单个核细胞45例,根据ACD与全血的体积比按1∶10、1∶12、1∶14分成3组,循环血量9000~12000 mL,血流速40~60 mL/min,观察3种不同ACD与全血体积比对采集前后血红蛋白及血小板、电解质Ca^2+、K^+、Na^+、Cl^-差异及采集过程中的变化.结果:采集前后血红蛋白质量浓度与Na^+、Cl^-浓度下降不显著,不同组间均无统计学差异(P>0.05);血小板与Ca 2+、K+下降有统计学差异(P<0.05),不同组间Ca^2+浓度下降、1∶10组低钙反应发生较其他两组明显,均有统计学差异(P<0.05);而血小板与K+浓度下降不同组间均无统计学差异(P>0.05).1∶14组出现2例供者采集管流出口小血栓形成.结论:采集前后血小板、Ca^2+、K^+浓度下降有显著差异,不同组间Ca^2+浓度下降有差异,而血红蛋白质量浓度、Na^+、Cl^-浓度采集前后无明显变化,组间无显著差异;ACD与全血体积比为1∶12时低钙反应发生较低且较安全.Objective:To observe the effects of different blood preservation solution(I)ACD/whole blood ratios on the blood,ions and safety before and after collecting peripheral blood mononuclear cells by AutoMNC program of COM.TEC blood cell separator.Methods:A total of 45 donors peripheral blood mononuclear cells mobilized by granulocyte colony-stimulating factor were collected by a COM.TEC blood cell separator.The ACD/whole blood ratios were divided into three groups(i.e.,1∶10,1∶12,and 1∶14)according to the ratio of ACD/whole blood.The circulating blood volume was 9000~12000 mL,and the blood flow rate was 40~60 mL/min.The changes of hemoglobin and platelets,electrolyte Ca^2+,K^+,Na^+,Cl^- and the changes were observed during the collection.Results:There was no significant decrease in hemoglobin,Na^+ and Cl^-before and after the collection,with no statistical difference between different groups(P>0.05).There was a significant decrease in platelets,Ca^2+,and K+before and after collection(P<0.05).The incidence of hypocalcemia in the 1∶10 group was significantly higher than that in the other two groups(P<0.05).However,there was no significant difference between different groups in the decrease of platelet and K+(P>0.05).Two cases of small thrombus were found in the outflow of donor collection tube in the 1∶14 group.Conclusion:There were significant differences in the decrease of platelet,Ca^2+ and K^+ before and after collection.There was a different decrease of Ca^2+ between groups,whereas there were no significant changes in hemoglobin,Na^+ and Cl^-,withoutstatistical difference between groups.Hypocalcemia reaction was lower and safer when ACD/whole blood ratio was at 1∶12.
