小切口开窗髓核摘除术治疗腰椎间盘突出症效果的临床研究

摘　　要：目的:探讨小切口开窗髓核摘除术治疗腰椎间盘突出症的效果。方法:回顾性分析本院2015年6月-2017年6月手术治疗的64例腰椎间盘突出症患者临床资料,按随机数字表法分为对照组与研究组,每组32例。对照组采用传统手术,研究组采用小切口开窗髓核摘除术手术。比较两组切口长度、手术时间、术中出血量、治疗效果、日本骨科学会评分(JOA)及视觉模拟评分法(VAS)评分。结果:研究组切口长度、手术时间及术中出血量均低于对照组(P<0.05);研究组治疗效果优良率高于对照组(P<0.05);研究组术后JOA评分高于对照组、而VAS评分低于对照组(P<0.05)。结论:小切口开窗髓核摘除术创伤小,既能解决椎间盘突出问题,又能减少对脊椎后方组织的破坏,维持脊柱的动力性稳定,是治疗腰椎间盘突出症极其可行的微创手术方法。Objective:To investigate the effect of small incision fenestration of remove the nucleus pulposus in the treatment of lumbar intervertebral disc herniation.Method:The clinical data of 64 patients with lumbar intervertebral disc herniation treated by surgery in our hospital from June 2015 to June 2017 were retrospectively analyzed,they were divided into control group and research group according to the random number table method,32 cases in each group.The control group was treated with conventional surgery,the research group was treated with small incision fenestration of remove the nucleus pulposus.The incision length,surgery time,intraoperative blood loss,treatment effect,Japanese orthopaedic association scores (JOA) and visual analogue score (VAS) were compared between the two groups.Result:The incision length,surgery time and intraoperative blood loss in the research group were lower than those in the control group (P<0.05).The excellent and good rate of treatment effect of the research group was higher than that of the control group (P<0.05).The postoperative JOA score of the research group was higher than that of the control group,while the VAS score was lower than that of the control group (P<0.05).Conclusion:Small incision fenestration of remove the nucleus pulposus has less trauma,which can not only solve the problem of disc herniation,but also reduce the damage to the posterior tissues of the spine and maintain the dynamic stability of the spine.It is an extremely feasible minimally invasive surgical method for the treatment of lumbar intervertebral disc herniation.
