椎间孔镜下球囊扩张减压治疗退变性腰椎椎间孔及侧隐窝狭窄的临床研究

摘　　要：目的:探讨椎间孔镜下球囊扩张减压术对退变性腰椎椎间孔及侧隐窝狭窄患者的治疗效果。方法:回顾性分析2017年1月-2018年1月在本院接受治疗的60例退变性腰椎椎间孔及侧隐窝狭窄患者的临床资料,患者均需接受椎间孔镜下球囊扩张减压术治疗,分析术后治疗效果、椎间孔高度、侧隐窝宽度、椎间孔面积、下肢疼痛程度、腰椎功能障碍指数情况。结果:术后12个月,60例患者中,优32例,良25例,中3例,优良率为95.00%;术后各时间段,患者椎间孔高度与侧隐窝宽度均高于术前,差异均有统计学意义(P<0.05);术后各时间段,患者健侧、患侧椎间孔面积比较,差异均无统计学意义(P>0.05);术后12个月,患者下肢疼痛程度明显低于术前,各项目ODI评分均低于术前,差异均有统计学意义(P<0.05)。结论:退变性腰椎椎间孔及侧隐窝狭窄患者经椎间孔镜下球囊扩张减压术治疗,效果显著,可有效缓解疼痛,减轻患者功能障碍,利于改善病变椎间孔面积。Objective: To investigate the effect of balloon dilatation and decompression under intervertebral foramen in the treatment of degenerative lumbar intervertebral foramen and lateral recess stenosis. Method: The clinical data of 60 patients with degenerative lumbar foramen and lateral recess stenosis who were treated in our hospital from January 2017 to January 2018 were retrospectively analyzed. All patients need to receive balloon dilatation and decompression under intervertebral foramen. The effect of postoperative treatment, height of intervertebral foramina, width of lateral recess, area of intervertebral foramina, pain degree of lower extremity and lumbar dysfunction index were analyzed. Result: 12 months after the operation, among the 60 patients, 32 cases were excellent, 25 cases were good, and 3 cases were moderate, with an excellent and good rate of 95.00%. The height of the intervertebral foramen and the width of the lateral recess were higher than those before operation, the differences were statistically significant(P<0.05). There were no significant differences in the area of intervertebral foramina between the healthy side and the affected side(P>0.05). 12 months after the operation, the lower extremity pain of the patient was significantly lower than that before operation, and the ODI score of all items were lower than those before operation, the differences were statistically significant(P<0.05). Conclusion: Patients with degenerative lumbar intervertebral foramen and lateral recess stenosis are treated with balloon dilatation and decompression under intervertebral foramen lens, and the effect was significant, it can effectively relieve the pain, relieve the dysfunction of patients, and improve the area of the intervertebral foramen.
