局部围刺联合针刀松解夹脊穴治疗带状疱疹后遗神经痛的临床效果观察

摘　　要：目的:观察局部围刺联合针刀松解夹脊穴治疗带状疱疹后遗神经痛的临床效果。方法:选取2016年6月-2019年4月于湖南中医药大学第二附属医院就诊的60例带状疱疹后遗神经痛患者。按随机数字表法分为观察组和对照组,每组30例。观察组采用局部围刺联合针刀松解夹脊穴治疗,对照组采用局部围刺联合针刺夹脊穴治疗。比较两组治疗前后简化McGill疼痛量表和临床效果。结果:治疗前,两组疼痛分级指数(PRI)、视觉模拟量表(VAS)及现时疼痛程度(PPI)评分比较,差异均无统计学意义(P>0.05)。治疗后,两组PRI、VAS及PPI均低于治疗前(P<0.05)。治疗后,观察组PRI、VAS及PPI均低于对照组(P<0.05)。观察组治疗总有效率高于对照组(P<0.05)。结论:局部围刺联合针刀松解夹脊穴对治疗带状疱疹后遗神经痛效果优于围刺联合针刺夹脊穴,可减轻患者疼痛,值得临床应用。Objective: To observe the effect of local peri-acupuncture combined with acupotomy to release Jiaji point in the treatment of postherpetic neuralgia. Method: From June 2016 to April 2019, 60 patients with postherpetic neuralgia in the Second Affiliated Hospital of Hu’nan University of Traditional Chinese were selected. According to the random number table method, they were divided into observation group and control group, 30 cases in each group. The observation group was treated with local peri-acupuncture combined with acupotomy to release Jiaji point, while the control group was treated with local peri-acupuncture combined with acupuncture Jiaji point. Simplified McGill pain scale and efficacy before and after treatment were compared between the two groups. Result: Before treatment, there were no statistically significant differences in pain grading index(PRI), visual analogue scale(VAS) and present pain intensity(PPI) scores between the two groups(P>0.05). After treatment, PRI, VAS and PPI in both groups were lower than those before treatment(P<0.05). After treatment, PRI, VAS and PPI in the observation group were lower than those in the control group(P<0.05). The total effective rate in the observation group was higher than that in the control group(P<0.05). Conclusion: The effect of local peri-acupuncture combined with acupotomy to release Jiaji point is better than local peri-acupuncture combined with acupuncture Jiaji point in the treatment of postherpetic neuralgia, it can alleviate the patient pain, which worthy of clinical application.
