脂肪组织血管基质移植联合补元复骨汤治疗骨折不愈合的临床研究

摘　　要：目的:探讨自体脂肪组织中血管基质组分移植联合中药补元复骨汤治疗骨折不愈合的临床疗效。方法:选取2017年6月-2018年11月收治的骨折不愈合患者28例,根据治疗方式的不同平均分为对照组(常规治疗)和观察组(自体脂肪组织中血管基质组分移植联合中药补元复骨汤治疗),各14例。观察组与对照组年龄、性别、骨折类型无显著差异(P>0.05)。术后定期拍片及抽血随访,重点观察两组临床疗效、患肢的骨痂出现时间、血清碱性磷酸酶和钙磷乘积。签署知情同意书,并经医院伦理委员会批准。结果:所有病例获得随访,观察组的临床疗效优良率为92.85%(13/14),明显高于对照组的57.14%(8/14)(P<0.05)。对照组骨痂出现时间为(11.56±2.98)周,观察组骨痂出现时间为(7.12±2.56)周,差异有统计学意义(P<0.05)。对照组血清碱性磷酸酶为(172.58±36.65)U/L,血钙磷乘积为(3.69±0.58)mmol/L;观察组血清碱性磷酸酶为(206.92±29.91)U/L,血钙磷乘积为(4.80±0.62)mmol/L,观察组均高于对照组,组间比较,差异有统计学意义(P<0.05)。结论:自体脂肪组织中血管基质组分移植联合中药补元复骨汤可有效治疗骨折不愈合。Objective To investigate the clinical efficacy of vascular matrix component transplantation combined with traditional Chinese medicine Buyuan Fugu decoction in the treatment of fracture nonunion.Methods Twenty-eight patients with nonunion of fracture from June 2017 to November 2018 were enrolled.All patients were divided into control group(conventional treatment group)and observation group(vascular matrix component transplantation combined with traditional Chinese medicine Buyuan Fugu decoction treatment group),14 cases each group.There was no significant difference in age,gender and fracture type between the observation group and the control group(P>0.05).Postoperative regular X-ray check and Draw blood tests,focus on the clinical efficacy of the two groups of patients,the occurrence of poroma in the affected limb,serum alkaline phosphatase and calcium-phosphate products.All patients in this study signed informed consent and were approved by the hospital ethics committee.Results:All cases were followed up.The clinical cure rate of the observation group was 92.85%(13/14),which was significantly higher than that of the control group 57.14%(8/14)(P<0.05).The poroma appearance time of the control group was(11.56±2.98)weeks,that time of observation group was(7.12±2.56)weeks,and the difference was statistically significant(P<0.05).The serum alkaline phosphatase of the control group was(172.58±36.65)U/L,and the product of serum calcium and phosphorus was(3.69±0.58)mmol/L.The serum alkaline phosphatase of the observation group was(206.92±29.91)U/L,and the calcium-phosphate product was(4.80±0.62)mmol/L.Obviously the observation group was higher than the control group.The difference was statistically significant(P<0.05).Conclusion The transplantation of vascular matrix components in autologous adipose tissue combined with traditional Chinese medicine Buyuan Fugu decoction can effectively treat fracture nonunion.
