扶肾颗粒调节decorin干扰腹膜胶原稳定性的作用机制概述

摘　　要：肾脏疾病是导致全球死亡率逐年增加的一个重要原因,肾脏替代治疗方案的选择面临着新一轮的机遇和挑战。腹膜透析(peritoneal dialysis,PD)与血液透析(hemodialysis,HD)长期生存率并无显著差异,但腹膜透析能够使患者更多掌握自主权、减少医疗成本。然而中长期腹膜透析的阻碍是腹膜功能、结构的改变,即引起腹膜纤维化。核心蛋白聚糖(decorin,Dcn)不仅影响胶原纤维的合成,而且干扰胶原纤维稳定性,Dcn还具有抗粘附分子的作用,能与许多其他ECM成分相结合(如FN和TSP-1),从而参与细胞增殖、迁移的调节。扶肾颗粒通过调控Dcn,抑制TGF-β通路的激活,干扰细胞外基质积聚、调节糖脂代谢从而抑制腹膜胶原形成,防止腹膜纤维化,对于阐明中医药防治腹膜间皮细胞EMT的作用机理,延缓腹膜纤维化,提高中长期腹透疗效具有重要意义。Kidney disease is a major cause of the increase in global mortality,and the choice of alternative treatment for kidneys faces new opportunities and challenges.There is no significant difference between the long-term survival rate of peritoneal dialysis(PD)and hemodialysis,but peritoneal dialysis can enable patients to gain more autonomy and reduce treatment costs.However,the barrier to long-term peritoneal dialysis is changes in peritoneal structure and function,cause peritoneal fibrosis.Dcn may not only affect the formation of collagen,but also interfere with the stability of collagen in the tissue.Dcn also has an anti-adhesion molecule and can bind to many other ECM components(such as FN and TSP-1).So participate in the regulate of cell proliferation and migration.Fushen granules regulate Dcn and inhibit the activation of the TGF-βpathway,interfere with the accumulation of extracellular matrix,regulate glycolipids metabolism to inhibit the formation of peritoneum collagen,soprevente peritoneal fibrosis,and clarify the prevention and treatment of peritoneal mesothelial EMT.It is of great significance to delay peritoneal fibrosis and improve the curative effect of longterm PD.
