摘 要:目的探讨宝石能谱CT不同成像技术在去除人体金属植入物伪影中的应用价值。方法对50例存在金属植入物的患者进行宝石能谱CT扫描,扫描后获得混合能量(140 kVp)图像。所有图像均于110 keV下并行重建,获得单能量(110 keV)+金属伪影消除技术(MARs)图像。由2名医生对图像的质量进行评价并比较混合能量图像和单能量+MARs图像的质量。结果不同评价者对图像质量的评价一致性非常好(Kappa值=0.824,P<0.05)。混合能量和单能量+MARs图像重建技术所获得的图像质量均能满足影像诊断的需要,但单能量+MARs获得的图像质量评分高于混合能量技术(P<0.05)。结论与混合能量成像比较,宝石能谱CT单能量+MARs不仅能够最大限度地消除金属植入物伪影带来的影响,还能兼顾对临近组织的显示,较好地满足临床诊断的需求。Objective To explore the application value of gemstone spectrum CT of different imaging modalities in reducing the artifacts caused by metal implants in the body.Methods Fifty patients with metal implants received gemstone spectrum CT scanning.After scanning,polychromatic images(140 kVp)were obtained.All the images received reconstruction in parallel at 110 keV to obtain monochromatic(110 keV)+metal artifact reduction system(MARs)images.The quality of the reconstructed images was evaluated by two doctors,and then polychromatic images and monochromatic+MARs images were compared with respect to image quality.Results An excellent consistency for assessment of image quality was found between different evaluators(Kappa value=0.824,P<0.05).Both polychromatic imaging and monochromatic+MARs imaging reconstruction technology could provide images with the quality eligible for imaging diagnosis,however,monochromatic+MARs technology achieved a higher score for image quality than polychromatic technology(P<0.05).Conclusion Compared with polychromatic imaging,gemstone spectrum CT with monochromatic+MARs can not only eliminate the influence of metal implants artifacts to the greatest extent,but also take the display of adjacent tissues into account,which better satisfies the requirements in clinical diagnosis.
