摘　　要：目的探讨团队模式院内血糖管理联合网络血糖监测系统对胃肠外科高血糖患者血糖控制水平的影响。方法2017年6月1日至11月30日入院的高血糖患者244例为团队管理组,由院内血糖管理团队联合网络血糖监测系统对高血糖进行主动干预;2016年6月1日至11月30日入院的高血糖患者224例为常规会诊组,采用常规会诊模式进行管理。比较两组管理模式血糖水平的差异。结果与常规会诊组相比,团队管理组平均血糖值降低,高血糖发生率(血糖>7.8 mmol/L)及严重高血糖发生率(血糖>13.9 mmol/L)降低,目标血糖达标率提高,平均每人每日血糖检测次数增加,糖化血红蛋白检测率以及规律使用胰岛素的比例均有增加,差异有统计学意义(P<0.05);而低血糖发生率(血糖≤3.9 mmol/L)、临床显著低血糖发生率(血糖<3.0 mmol/L)及平均住院床日比较,差异无统计学意义(P>0.05)。结论团队模式院内血糖管理联合网络血糖监测系统有效改善了胃肠外科高血糖患者的血糖控制水平。Objective To investigate the effect of team mode of hospital blood glucose management combined with internet-based glucose monitoring system on blood glucose control status in patients with hyperglycemia at department of gastroenterological surgery.Methods The study included 244 hyperglycemia patients from June 1,2017 to November 30,2017 at department of gastroenterological surgery(team management group).The patients underwent active intervention by using team mode of hospital blood glucose management combined with internet-based glucose monitoring.Another 224 hyperglycemia patients from June 1,2016 to November 30,2016 at department of gastroenterological surgery were treated with routine consultation control mode(routine consultation group).The difference in blood glucose levels between the two groups was compared.Results Compared with the routine consultation group,the team management group had lower average blood glucose level,lower incidence of hyperglycemia(blood glucose>7.8 mmol/L)and lower incidence of severe hyperglycemia(blood glucose>13.9 mmol/L),higher control rate of target blood glucose,increased average number of blood glucose measurement per person per day,higher detection rate of glycated hemoglobin and higher proportion of patients who regularly used insulin.The differences were statistically significant(P<0.05).There were no significant differences in the incidence of hypoglycemia(blood glucose≤3.9 mmol/L),the incidence of clinical significant hypoglycemia(blood glucose<3.0 mmol/L)and the average hospital length of stay between the two groups(P>0.05).Conclusion The team mode of hospital blood glucose management combined with internet-based glucose monitoring system could effectively improve the situation of hyperglycemia patients at department of gastroenterological surgery.
