间断式褥式缝合法在跟骨骨折术后切口愈合及体位摆放对患肢肿胀影响观察

摘　　要：目的观察间断式褥式缝合法在跟骨骨折术后切口愈合及体位摆放对患肢肿胀影响。方法选取我院收治的跟骨骨折患者102例,根据随机及患者自愿原则将其分为常规组(n=55)和改进组(n=47),常规组进行间断垂直褥式缝合,改进组进行改良间断垂直褥式缝合,比较两组患者手术相关指标(伤口出血量、伤口周围红肿、伤口愈合时间、引流时间)、切口愈合效果(甲级、乙级、丙级)、术后疼痛评分以及肿胀评分(视觉模拟评分)、临床疗效以及术后并发症发生情况。结果改进组伤口愈合时间短于常规组,伤口周围红肿例数少于常规组,术后切口甲级愈合率高于常规组,切口丙级愈合率低于常规组(P<0.05);两组伤口出血量、引流时间比较,差异无统计学意义(P>0.05);术后1 d,两组疼痛评分及肿胀评分比较,差异无统计学意义(P>0.05),术后3、7 d,改进组疼痛评分及肿胀评分低于常规组(P<0.05);改进组术后优良率、并发症发生率优于常规组(P<0.05)。结论改良间断式褥式缝合法治疗跟骨骨折,可有效促进患者术后切口愈合,减轻体位摆放期间患肢疼痛、肿胀,降低并发症发生。Objective To observe the effects of intermittent mattress suture in postoperative incision healing of calcaneal fracture and position display on swelling of affected limbs.Methods A total of 102 patients with calcaneal fractures treated in our hospital were divided into routine group(n=55)and modified group(n=47)according to the randomization and patient voluntary principles.The routine group was given intermittent vertical mattress suture while the modified group was given modified intermittent vertical mattress suture.Surgical related indexes(wound blood loss,redness and swelling around the wound,wound healing time,and drainage time),incision healing effects(grade A,B and C),postoperative pain score and swelling score(visual analogue scale),clinical efficacy and occurrence of postoperative complications were compared between the two groups.Results The wound healing time in the modified group was shorter than that in the routine group(P<0.05),and the number of cases with redness and swelling around the wound was less than that in routine group(P<0.05).There was no significant differences in the wound blood loss and drainage time between the two groups(P>0.05).The grade A healing rate of postoperative incision was significantly higher while the grade C healing rate was significantly lower in the modified group than those in the routine group(P<0.05).After 1 day of surgery,there was no statistically significant differences in the pain score and swelling score between the two groups(P>0.05).After 3 and 7 days of surgery,the pain score and swelling score in the modified group were significantly lower than those in the routine group(P<0.05).The postoperative excellent and good rate and incidence rates of complications in the modified group were significantly better than those in the routine group.Conclusion Modified intermittent mattress suture for calcaneal fractures can effectively promote the postoperative incision healing,relieve the pain and swelling of affected limb during position display,and reduce the occurr
