摘  要:目的对辽宁省2017—2019年抗原性发生改变的甲型H1N1流感毒株进行耐药性及血凝素(hemagglutinin,HA)基因分析,了解其耐药性及抗原性变化情况,为流感的防控及临床治疗提供依据。方法选取275株甲型H1N1流感毒株进行抗原性分析,将低反应株进行神经氨酸酶(neuraminidase,NA)抑制剂类药物耐药性检测及HA基因分析。结果本研究共检测到参考株的低反应株32株,经NA抑制剂类药物耐药性分析,10株毒株对NA抑制剂类药物奥司他韦敏感性降低。部分毒株HA蛋白中涉及抗原决定簇Sa和Ca1的多个位点发生突变。8株毒株糖基化位点与当年疫苗参考株存在差异。检出1株毒株位于220环的氨基酸位点发生R222K的突变,提示蛋白结构可能会发生变化,导致流感病毒的受体结合能力改变。结论流感病毒的抗原性对于病毒的致病性及传播均具有重要的影响,关注其发生改变的毒株,对流感病毒的及时防控及临床治疗具有重要的实践意义及指导作用。Objective To analyze the antiviral resistance and hemagglutinin (HA) gene of the influenza A (H1N1)pdm09 strains with antigenicity mutations in Liaoning province from 2017 to 2019, so as to understand the changes in the antiviral resistance and antigenicity and to provide evidences for the prevention and control as well as clinical treatment of influenza.Methods Antigenicity analysis was performed for 275 influenza A (H1N1)pdm09 strains. The low-response strains were tested for resistance to neuraminidase (NA) inhibitors and the HA genes were analyzed.ResultsIn this study, 32 low-response strains comparing to the reference strains were detected. The analysis of resistance to NA inhibitors showed that ten stains were less sensitive to oseltamivir. Mutations in the antigen-determining clusters Sa and Ca1 of HA in some strains were observed. The glycosylation sites of eight strains were different from the reference strains of the corresponding years. The amino acid mutation of R222K in loop 220 of one strain was detected, suggesting possible change in protein structure, leading to the change in binding ability of the influenza virus to the receptor.Conclusions As the antigenicity of influenza virus has an important impact on the pathogenicity and transmission of the virus, attention to the changed strains is greatly significant for timely prevention and control as well as clinical treatment of influenza virus.
