分别基于胫骨与股骨为参考点的MOWHTO术后髌骨高度测量及其与前方截骨间隙的相关性分析

摘　　要：目的:分别以胫骨与股骨为参考点,应用Caton指数、FPHI指数测量胫骨高位内侧开放楔形截骨术(MOWHTO)后髌骨高度的变化,并探讨前方截骨间隙与髌骨高度变化的相关性。方法:选择行MOWHTO的73例患者(共82膝),术后1周内均行膝关节CT重建,测量胫骨结节后内方截骨间隙。并以此分为2组:A组<10 mm,36例39膝;B组≥10 mm,37例43膝。分别应用Caton指数及FPHI指数测量2组术前及术后3个月时的髌骨高度,比较2组术前及术后髌骨高度的改变。结果:以Caton指数为测量指标,2组组内术前与术后3个月比较差异均有统计学意义(均P<0.05);以FPHI指数为测量指标,A组术前与术后3个月差异无统计学意义(P>0.05),B组差异有统计学意义(P<0.05)。结论:MOWHTO术后髌骨高度受前方截骨间隙影响,当间隙≥10 mm时影响会更加显著。Objective:With tibia and femur as reference,the changes of patellar height after medial open wedge high tibial osteotomy(MOWHTO)were measured.The correlation between the anterior gap of osteotomy and patellar height was discussed.Methods:A total of 73 patients with 82 knees who underwent MOWHTO were included this retrospective study.CT was reconstructed within one week after operation.The anterior gap of osteotomy was measured and divided into two groups.Group A was less than 10 mm and Group B was more than or equal to 10 mm.The Caton index and FPHI index were used to measure the patellar height before and 3 months after operation.The changes of patellar height in Group A and Group B were compared.Results:Group A was 39 cases and Group B was 43 cases.In terms of Caton index,there were significant differences in Group A and Group B.In terms of FPHI index,there was no significant difference in Group A,but significant difference in Group B.Conclusions:The patellar height of MOWHTO is affected by the anterior gap of osteotomy,which is more significant when the gap is more than or equal to 10 mm.
