摘 要:目的观察2型糖尿病(T2DM)周围神经病变患者外周血维生素B 1(VitB 1)水平变化,分析VitB 1与糖尿病周围神经病变(DPN)及其他临床指标的相关性。方法选取2017年9月至2018年5月就诊于河南省人民医院的354例T2DM患者。根据DPN的诊断标准将研究对象分为单纯糖尿病组(Non-DPN组,237例)和糖尿病周围神经病变组(DPN组,117例)。收集两组患者的一般情况,检测生化指标及维生素水平。比较两组患者血清VitB 1水平与双侧腓肠神经的感觉神经传导速度(SNCV)、双侧胫后神经的运动神经传导速度(MNCV)及其他各临床指标的相关性;应用Logistic回归模型分析DPN的危险因素。结果Non-DPN组与DPN组VitB 1、VitB 6、VitB 12、糖化血红蛋白(HbA1c)、总胆固醇(TC)及尿微量白蛋白/肌酐比(A/C)水平比较,差异有统计学意义(P<0.05)。血清VitB 1水平与HbA1c、总白细胞数量、间接胆红素水平呈显著负相关,与腓肠神经的SNCV、胫后神经的MNCV、VitB 6、VitB 9及VitB 12水平呈显著正相关(P<0.05)。Logistic回归模型显示,VitB 1和VitB 12是DPN的保护因素,病史和HbA1c是DPN的危险因素。结论血清VitB 1水平与T2DM神经病变密切相关,VitB 1是DPN的独立保护因素。Objective To observe the change of vitamin B 1(VitB 1)in type 2 diabetes(T2DM)with peripheral neuropathy and analyze the correlation of VitB 1 with diabetic peripheral neuropathy(DPN)and other clinical indicators.Methods A total of 354 T2DM patients were selected from Henan Provincial People’s Hospital from September 2017 to May 2018.According to the diagnostic criteria of DPN,all the objects were divided into simple diabetes group(Non-DPN group,237 cases)and DPN group(117 cases).The general conditions were obtained.Biochemical indicators and vitamin levels were tested.The correlations of VitB 1 with the sensory nerve conduction velocity(SNCV)of sural nerve,the motor nerve conduction velocity(MNCV)of posterior tibial nerve and other clinical index were evaluated.The risk factors of DPN were analyzed by Logistic regression model.Results The levels of VitB 1,VitB 6,VitB 12,Glycosylated hemoglobin(HbA1c),total cholesterol(TC)and urine A/C showed significant differences between Non-DPN group and DPN group(P<0.05).The level of VitB 1 was negatively correlated with HbA1c,total white blood cell count and indirect bilirubin level.It was significantly positively correlated with the SNCV of sural nerve,the MNCV of posterior tibial nerve,the level of VitB 6,VitB 9 and VitB 12(P<0.05).Logistic regression model showed that VitB 1 and VitB 12 were protective factors for DPN,diabetic history and HbA1c were risk factors for DPN.Conclusion The serum level of VitB 1 is closely related to the peripheral neuropathy of T2DM and VitB 1 is the independent protective factor of DPN.
