脑室内室管膜下瘤MRI诊断与鉴别诊断

摘　　要：目的:探讨室管膜下瘤MRI影像特征,提高对室管膜下瘤诊断水平及术前诊断准确率。方法:选择病理证实为室管膜下瘤患者13例,术前均行颅脑MRI平扫及增强扫描,回顾性分析MRI表现,总结影像学特点。结果:13例室管膜下瘤均为单发,其中第四脑室4例,侧脑室9例。MRI平扫表现为脑室内不规则孤立肿块,边缘清楚,信号不均匀。增强扫描中11例病灶无明显强化;2例病灶内见斑点状、斑片状轻度强化。结论:室管膜下瘤在临床中少见,室管膜下瘤MRI增强检查多数病灶不强化或仅有斑点状、斑片状轻度强化,与其他脑室内肿瘤明显强化不同,具有一定影像学特点,故颅脑MRI平扫+增强检查对其术前诊断有较高价值。Objective:To investigate the mri imaging features of subependymomas,so as to improve the diagnosis of subependymomas and improve the accuracy of preoperative diagnosis.Methods:All 13 patients were collected,which were biopsy confirmed as subependymomas.All patients underwent routine mri scan and enhanced scan before operation.The mri findings were retrospectively analyzed and their imaging features were summarized.Results:All 13 cases of subependymomas were single,including four cases of fourth ventricle and nine cases of lateral ventricle.mri plain scan showed irregular isolated mass in the ventricle with clear edges and uneven signal.there were no obvious enhancements in 11 lesions in the enhanced scan,and there were spotted and patchy mild enhancement in 2 lesions.Conclusion:Subependymomas are rare in clinical work.most of the lesions are not enhanced or only spotted and patchy mildly enhanced.they are different from other intraventricular tumors.learning characteristics,so the brain mri scan and enhanced examination has a relatively high value for preoperative diagnosis.
