摘　　要：目的探讨彩色多普勒血流成像对旋髂浅动脉的显示价值,为旋髂浅动脉皮瓣的临床应用提供影像学信息。方法对60例拟取旋髂浅动脉皮瓣行重建术患者的旋髂浅动脉及其分支血管采用CDFI进行检测,观察旋髂浅动脉的走行位置,是否存在浅支,记录旋髂浅动脉的直径,距离皮肤的深度,并测量旋髂浅动脉的峰值流速及阻力指数等。结果彩色多普勒超声可清晰显示旋髂浅动脉的位置、直径及其血流信号;对于大部分患者可清晰显示其浅支。60例中检测到旋髂浅动脉深支60例,显示率100%;旋髂浅动脉浅支41例,显示率68.3%。超声对于旋髂浅动脉均做出了准确的评估并体表定位,与术中所见吻合。结论彩色多普勒超声能清晰地显示旋髂浅动脉并对其进行准确的术前评估,为皮瓣的设计提供影像学依据,具有重要的指导意义及应用价值。Objective To evaluate the value of color Doppler flow imaging in the display of the superficial circumflex iliac artery,and to provide imaging information for the clinical application of the flap.Methods The superficial iliac circumflex artery and its branches were examined by CDFI in 60 patients who were going to take the skin flap of superficial iliac circumflex artery for reconstruction.We observed the location of superficial circumflex iliac artery and whether there was a superficial branch,then,recorded the diameter,the depth from the skin and the peak velocity and resistance index of the superficial circumflex iliac artery.Results Color Doppler flow imaging could not only show the location,diameter and blood flow signal of the superficial iliac circumflex artery clearly,but also display the superficial branches of them.Sixty deep branches of superficial circumflex iliac artery out of 60 patients were detected,whose display rate was 100%.Forty-one superficial branches of superficial circumflex iliac artery out of 60 patients were detected,whose display rate was 68.3%.Ultrasonography made an accurate evaluation of the superficial iliac circumflex artery and located it on the body surface,which was consistent with what was seen during the operation.Conclusion Color Doppler flow imaging can clearly show the superficial iliac circumflex artery and accurately evaluate it before operation.It provides imaging basis for flap design,which has important guiding significance and application value.
