摘　　要：目的分析促性腺激素释放激素类似药物(GnRHa)治疗特发性中枢性性早熟(ICPP)女性儿童前后血清脂联素(ADPN)水平变化、影响因素及其意义。方法选取50例ICPP女童为研究对象,均进行GnRHa治疗,剂量每月60~100μg/kg。将治疗前设为对照组,治疗3个月设为观察Ⅰ组,治疗6个月设为观察Ⅱ组。记录并比较治疗前(入组当天)和治疗后(治疗3、6个月)患儿身高、体重、ADPN等相关指标,观察骨龄、卵巢和子宫变化情况。结果治疗前,肥胖或超重患儿ADPN水平低于正常体格患儿(P<0.05);随着治疗进行,患儿BMI下降,血糖、血脂及甲状腺功能等指标趋于正常,ADPN水平明显升高,观察Ⅱ组高于观察Ⅰ组和对照组,差异有统计学意义(P<0.05);不同治疗时期患儿骨龄有所增加,卵巢和子宫容积减小,但均变化不明显(P>0.05)。结论应用GnRHa治疗期间ICPP患儿ADPN明显升高,糖脂代谢水平和发育逐渐恢复正常;ADPN可作为ICPP临床检验新的脂代谢指标,为其治疗效果评估提供参考依据。Objective To investigate the Changes of serum adiponectin ADPN in woman children with idiopathic central precocious puberty(ICPP)before and after treatment by gonadotropin releasing hormone similar drugs(GnRHa),and to explore its influencing factors and significance.Methods A total of 50 woman children with ICPP were enrolled in the study,who were treated by GnRHa,with a dose of 60~100μg/kg.These patients were divided into control group(before treatment),observation groupⅠ(3-momth treatment)and observation groupⅡ(6-momth treatment).The patient’s body height and body weight,ADPN levels,the changes of skeletal age,ovary and uterus were observed and compared among the three groups.Results Before treatment,ADPN levels in obese or overweight patients were significantly lower than those in healthy children(P<0.05).With the treatment,the BMI was decreased,and blood glucose,blood lipids and thyroid function indexes were returned to normal levels,however,ADPN levels were increased significantly,moreover,which in observation groupⅡwere significantly higher than those in observation groupⅠ(P<0.05).Moreover the skeletal age was increased,and ovary and uterus volume were decreased at different treatment periods,however,the changes were not significant(P>0.05).Conclusion During the treatment by GnRHa,ADPN levels in woman children are significantly increased,and glucose and lipid metabolism levels as well as development are gradually returned to normal levels.ADPN can be regarded as an new index in clinical detection,which can provide reference for the evaluation of therapeutic effects.
