摘 要:目的探讨慢性乙型肝炎(CHB)患者血清α1微球蛋白(α1-MG)与乙型肝炎病毒DNA(HBV-DNA)载量的相关性。方法收集233例CHB患者血清。将CHB患者根据乙型肝炎病毒表面抗原(HBsAg)、乙型肝炎病毒e抗原(HBeAg)分为HBsAg阳性患者(190例)和HBeAg阳性患者(43例)。分别测定其α1-MG和HBV-DNA载量,采用免疫比浊法检测血清α1-MG;采用荧光定量PCR法检测血清HBV-DNA载量,分析血清α1-MG水平与血清HBV-DNA载量的相关性。结果HBsAg阳性患者中HBV-DNA高载量患者88例(高载量A组),HBV-DNA低载量患者102例(低载量A组)。高载量A组α1-MG水平为(26.5±8.5)mg/L,低载量A组α1-MG水平为(20.5±5.5)mg/L,两组比较,差异无统计学意义(t=1.1057,P>0.05)。HBeAg阳性患者HBV-DNA载量分析结果显示,HBV-DNA高载量患者15例(高载量B组),HBV-DNA低载量患者28例(低载量B组)。高载量B组α1-MG水平为(19.2±7.2)mg/L,低载量B组α1-MG水平为(18.5±6.3)mg/L,差异无统计学意义(t=1.3251,P>0.05)。结论血清α1-MG水平与HBV-DNA载量无明显的相关性,不能用于判断CHB患者病毒复制情况,对病变的严重程度、观察疗效及评估预后无指导作用。Objective To discuss the correlation of HBV-DNA load and level ofα1 microglobulin(α1-MG)in patients with chronic hepatitis B(CHB).Methods A total of 233 CHB patients′serum were collected.According to results of hepatitis B virus surface antigen(HBsAg)and hepatitis B virus e antigen(HBeAg),patients were divided into HBsAg positive patients(n=190)and HBeAg positive patients(n=43).HBV-DNA load and level ofα1-MG were detected by immunoturbidimetry and fluorescence quantitative PCR.In addition,correlation of HBV-DNA load and level ofα1-MG was analyzed.Results In HBsAg positive patients,higher HBV-DNA load(higher load group A)had 88 cases and lower HBV-DNA load(lower load group A)had 102 cases.The level ofα1-MG of higher load group A and lower load group A were(26.5±8.5)mg/L and(20.5±5.5)mg/L,the difference had no statistically significance(t=1.1057,P>0.05).In HBeAg positive patients,higher HBV-DNA load(higher load group B)had 15 cases and lower HBV-DNA load(lower load group B)had 28 cases.The levels ofα1-MG of higher load group and lower load group B were(19.2±7.2)mg/L and(18.5±6.3)mg/L,the difference had no statistically significance(t=1.3251,P>0.05).Conclusion There is no correlation of level ofα1-MG and HBV-DNA load,which could not be used in estimating the viral replication.The degree of disease,curative effect and evaluation of prognosis would not rely on the two indicators.
