带状疱疹急性期患者血清前炎性细胞因子、免疫球蛋白和T淋巴细胞亚群水平与后遗神经痛的相关性分析

摘　　要：目的探讨血清前炎性细胞因子、免疫球蛋白及T淋巴细胞亚群在带状疱疹(HZ)急性期患者的水平及与后遗神经痛(PHN)的相关性。方法选取2016年1月至2017年12月确诊为HZ患者348例,患者随访3个月观察PHN情况,出现PHN的患者52例作为PHN组,未发生PHN的HZ患者296例作为HZ对照组,另选择同期体检正常的健康者50例作为健康对照组。酶联免疫法检测血清β-内啡肽(β-EP)、神经降压素(NT)、降钙素基因相关肽(CGRP)、血浆P物质(SP)和神经生长因子诱导蛋白(VGF);流式细胞仪检测细胞炎性因子,肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)、白介素-8(IL-8)、白介素-10(IL-10)、白介素-1β(IL-1β)及CD+3、CD+4、CD+8T淋巴细胞亚群。免疫散射比浊法检测血清免疫球蛋白C3、C4、IgG、IgA和IgM。结果3组血清疼痛介质两两比较差异均有统计学意义(P<0.05),其中PHN组和HZ对照组中β-EP、NT明显低于健康对照组,CGRP、SP及VGF明显高于对照组,差异有统计学意义(P<0.05),PHN组中β-EP、NT明显低于HZ对照组,CGRP、SP及VGF明显高于HZ对照组,差异有统计学意义(P<0.05);PHN组和HZ对照组中IL-6、TNF-α、IL-1β、IL-10及IL-8水平均明显高于健康对照组,差异有统计学意义(P<0.05),PHN组中IL-6水平明显高于HZ对照组,而TNF-α、IL-1β、IL-10及IL-8水平组间差异无统计学意义(P>0.05)。3组血清免疫球蛋白水平两两比较,差异均有统计学意义(P<0.05),其中PHN组和HZ对照组中C3、C4、IgG、IgM及IgA明显低于健康对照组,差异有统计学意义(P<0.05),PHN组中C3、C4、IgG、IgM及IgA明显低于HZ对照组,差异有统计学意义(P<0.05);PHN组和HZ对照组中CD+3、CD+4、CD+8均明显低于健康对照组,而CD+4/CD+8明显高于健康对照组,差异有统计学意义(P<0.05),而PHN组和HZ对照组差异无统计学意义(P>0.05)。Spearson相关性分析显示IL-6和β-EP、NT呈明显负相关(P<0.05),与CGRP、SP及VGF呈明显正相关(P<0.05);免疫球蛋白Objective To investigate the serum levels of proinflammatory cytokines,immunoglobulin and T lymphocyte subsets in patients with acute stage herpes zoster(HZ)and their correlation with neuropathic pain.Methods A total of 348 patients who were diagnosed as HZ from January 2016 to December 2017 were selected as the research objects.All the patients were followed up for 3 months to observe the condition of postherpetic neuralgia(PHN).The 52 patients who occurred PHN were enrolled as PHN group,and 296 patients who did not occur PHN were enrolled as HZ control group.At the same time,50 healthy subjects who underwent physical examination were served as health control group.The serum levels ofβendorphin(βEP),neurotensin(NT),calcitonin generelated peptide(CGRP),plasma substance P(SP)and nerve growth factor inducible protein(VGF)were detected by ELISA.Moreover the levels of inflammatory factors,tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),interleukin-8(IL-8),interleukin 10(IL-10),interleukin-1β(IL-1β),and T lymphocyte subsets such as CD+3,CD+4,CD+8 were detected by flow cytometry.In addition the serum immunoglobulin C3,C4,IgG,IgA and IgM were detected by immunescattering turbidimetry.Results There were significantdifferences in the serum contents of pain mediatorsamong the three groups by pairwise comparison(P<0.05),in which,the levels ofβEP and NT in PHNgroup and HZ control group were significantly lowerthan those in health control group,moreover thelevels of CGRP,SP and VGF in PHN group and HZ control group were significantly higher than those in health control group(P<0.05).However the levels ofβEP,NT in PHN group were significantly lower than those in HZ control group,however,the levels of CGRP,SP and VGF in PHN group were significantly higher than those in HZ control group(P<0.05).Moreover the levels of IL-6,TNF-α,IL-1β,IL-10 and IL-8 in PHN group and HZ control group were significantly higher than those in health control group(P<0.05).In addition the levels of IL-6 in PHN group were significantly high
