摘 要:目的评价不同剂量右美托咪啶对蛛网膜下腔出血急性期大鼠神经功能的影响。方法健康清洁级SD大鼠50只,雌雄不限,3个月龄,体重250~300 g,采用随机数字表法分为5组(n=10):对照组(C组)、蛛网膜下腔出血组(S组)、低剂量右美托咪啶组(D1组)、中剂量右美托咪啶组(D2组)、高剂量右美托咪啶组(D3组)。C组不泵注药物,S组、D1组、D2组和D3组分别于枕大池中注入1 ml/kg自体血,制做蛛网膜下腔出血大鼠模型。D1组、D2组和D3组造模完成即刻,分别经尾静脉泵注右美托咪啶(DEX)2.5μg·kg-1·h-1、5μg·kg-1·h-1和10μg·kg-1·h-1,S组泵注0.9%氯化钠溶液1.5 ml/h,持续2 h。造模后48 h,将5组大鼠随机分为2个亚组(n=5),一亚组进行神经行为学评分,记录各组大鼠神经行为学评分情况,然后处死取全脑,采用eliot干湿重法测量脑组织含水量;另一亚组直接处死,取脑组织,做石蜡切片,观察基底动脉变化,采用免疫组化法,检测海马组织TNF、BCL-2、Bax蛋白的表达情况。结果神经行为学评分:与C组相比,S、D1、D2、D3组评分升高(P<0.05),D1组与S组比较差异无统计学意义(P>0.05),D2、D3组评分低于S组(P<0.05)。脑水含量:与C组相比,S、D1、D2、D3组脑水含量高于对照组(P<0.05),D1、D2组与S组比较差异无统计学意义(P>0.05),D3组脑水含量低于S组(P<0.05)。基底动脉:C组,基底动脉内膜平滑,未见明显增生、褶皱,S组基底动脉内膜褶皱,管壁增厚,内膜细胞出现明显增生,D1、D2、D3基底动脉痉挛减轻,内膜出现轻微褶皱,未见明显增生;各组比较显示S、D1、D2、D3组TNF表达升高,其中D1组与S组相比差异无统计学意义(P>0.05),D2、D3组与S组相比,TNF表达减少(P<0.05);与S组相比D1、D2、D3组Bcl-2表达逐渐增高(P<0.05);与C组相比S、D1、D2组Bax表达升高,D1组与S组之间差异无统计学意义(P>0.05),D2、D3组表达较S组减少(P<0.05)。结论右美托咪啶可改善蛛网膜下腔出血急Objective To investigate the effects of different doses of dexmedetomidine on nerve function in rat with acute subarachnoid hemorrhage(SAH).Methods Fifty SD rats,both male and famle,aged 3 months,weighing 250-300g,were randomly divided into five groups:control group(group C),SAH group(group S);low dose of dexmedetomidine group(group D1),medium dose of dexmedetomidine group(group D2)and high dose of dexmedetomidine group(group D3),with 5 rats in each group.The rat models with SAH were established by injecting 1ml/kg autoblood blood into the cisterna magna.After the rat models with SAH were established immediately,the rats in group D1,group D2,group D3 were given dexmedetomidine by intravenous infusion via caudal vein,with the dose of 2.5μg·kg-1·h-1,5μg·kg-1·h-1 and 10μg·kg-1·h-1,respectively,however,the rats in group S were given 0.9%sodium chloride solution dexmedetomidine by intravenous infusion via caudal vein while normal saline 1.5ml/h were given with the dose of 1.5ml/h for 2 hours.At 48 hours after the operation the rats in the five groups were divided into five subgroups,with 5 rats in each group,the neurologic behavior scores were evaluated in one of the subgoups,and the rats were sacrificed,and the brain tissues were taken to detect the ratio of wet to dry by Eliot measurement method.The rats in the other subgroups were sacrificed,and the brain tissues taken for pathologic examination by light microscope and the detection of expression levels of TNF,Bax and Bcl-2 in hippocamus by immunohistochemisty.Results As compared with those in group C,the neurologic behavior scores in group S,group D1,group D2 and group D3 were significantly increased(P<0.05),which in D2 and group D3 were significantly lower than those in group S(P<0.05),however,there were no significant differences between group D1 and group S(P>0.05).The brain water contents in group S,group D1,group D2 andgroup D3 were significantly higher than those ingroup C(P<0.05),which in D were significantlylower than those in group S(P<0.05),howe
