摘 要:目的探讨595 nm脉冲染料激光和CO2激光治疗200例儿童蜘蛛痣的临床疗效及安全性。方法选取皮肤科2018年10月至2019年3月收治的200例蜘蛛痣患儿的临床资料,病变部位均为面部,其中皮损部位共405处,单个皮损患儿65例,2个及2个以上皮损135例。200例患儿随机分为595 nm脉冲染料激光组(595 nm组)与CO2组,每组100例。595 nm组采用Vbeam脉冲染料激光仪;CO2组采用CO2激光治疗仪。观察患儿的治疗次数、治疗一次及多次后的疗效、治疗过程中的不良反应及治疗结束后随访并发症发生情况。结果595 nm组患儿,其中皮损共211处,平均治疗次数为(1.20±0.67)次;CO2组患儿100例,其中皮损共194处,平均治疗次数为(1.95±0.71)次。2组患儿治疗次数差异有统计学意义(P<0.05)。595 nm组和CO2组患儿一次治愈率分别为88.15%、82.47%,差异有统计学意义(P<0.05);经多次治疗治愈率为95.73%、96.90%,差异无统计学意义(P>0.05)。595 nm组不良反应发生率9.0%、CO2组13.0%,差异有统计学意义(P<0.05);继续随访8周,部分出现色素减退、水泡及瘢痕的患儿,其皮损均逐渐消退,无永久不良反应。随访半年后未见复发。结论595 nm脉冲染料激光和CO2激光治疗儿童蜘蛛痣效果显著,易于操作,患儿接受程度高,且无明显不良反应,可在临床中首选使用。Objective To investigate the clinical efficacy and safety of 595nm pulsed dye laser and CO2 laser in treatment of 200 cases of spider nevus in children.Methods The clinical data of 200 children with spider nevus who were admitted and treated in our hospital from October 2018 to March 2019 were retrospectively analyzed.The lesions were all on the face of patients,and there were 405 skin lesions,in which,there were 65 cases of single lesion and 135 cases of more than two lesions.A total of 200 children patients were randomly divided into 595nm pulsed dye laser group(595nm group,n=100)and CO2 group(n=100).The patients in 595nm group were treated by Vbeam pulsed dye laser,however the patients in CO2 group were treated by CO2 laser treatment apparatus.After the treatment,the frequency of treatments,the therapeutic effects of once or many times,the adverse reactions during treatment and the complications after follow up were observed and compared between the two groups.Results There were 211 skin lesions in 595nm group,and the average number of treatment was(1.20±0.67).However there were 194 skin lesions in CO2 group,and the average number of treatment was(1.95±0.71),there was a significant difference in the number of treatment between the two groups(P<0.05).Moreover there was significant difference in the one-time cure rate between 595nm group and CO2 group(88.15%vs 82.4%,P<0.05).In addition there was no significant difference in the cure rate of multiple treatments between the 595nm group and CO2 group(95.73%vs 96.90%,P>0.05).However there was significant difference in the incidence rate of adverse reactions between the 595nm group and CO2 group(9.0%vs 13.0%,P<0.05).After 8-week follow-up,the skin lesions in some children with hypopigmentation,blisters and scars were gradually disappeared without permanent adverse reactions.No recurrence was found after 6-month follow-up.Conclusion The 595nm pulsed dye laser and CO2 laser are effective and easy to operate in treatment of spider nevus in children patient,with high a
