摘　　要：目的探讨不同心理状态对骨关节炎患者行全膝关节置换术的影响因素。方法选取2017年3月至2018年3月骨关节外科行全膝关节置换术的骨关节炎患者100例。采用问卷调查的方法,以及测评术前患者的心理状态,将患者按照心理正性心理组37例与负性心理组63例(术前采用正负情绪量表、汉密顿抑郁量表、SDS量表),明确不同心理状态下对骨关节炎患者行全膝关节置换术后的影响因素。结果2组患者正负情绪得分、HAMD、SDS差异有统计学意义(P<0.05)。正性心理组HSS评分中疼痛、功能、活动度、肌力、屈曲畸形、稳定性显著高于负性心理组差异有统计学意义(P<0.05)。正性心理组WOMAC骨关节炎评分中疼痛、功能、僵硬显著低于负性心理组差异有统计学意义(P<0.05)。结论不同心理状态下对于患者行全膝关节置换术后身体恢复具有显著影响,主要影响因素包括疼痛、功能、活动度、肌力、屈曲畸形、稳定性、僵硬程度等。Objective To investigate the influencing factors after total knee arthroplasty in patients with osteoarthritis under different psychological states.Methods A total of 100 patients with osteoarthritis who were treated by total knee arthroplasty in our hospital from March 2017 to March 2018 were enrolled in the study,who were divided into positive psychological group(n=37)and negative psychological group(n=63),and the questionnaire survey was performed to assess the psychological status of patients before operation by means of Hamilton depression scale and SDS scale.Mopreover the influencing factors on the patients with osteoarthritis after total knee arthroplasty under different psychological conditions were analyzed.Results There were no significant differences in patient’s age and sex,body weight,BMI,osteoarthritis grade and occupation between the two groups(P>0.05),however,there were significant differences in positive and negative emotional scores,HAMD and SDS between the two groups(P<0.05).The HSS scores including pain,function,range of motion,muscle strength,flexion deformity,stability,function,mobility,muscle strength,flexion deformity in positive psychological group were significantly higher than those in negative psychological group(P<0.05).However the WOMAC osteoarthritis score including pain,function,activity degree,muscle strength,flexion deformity and stability in positive psychological group were significantly lower than those in negative psychological group(P<0.05).Conclusion The different psychological states has significant effects on the recovery of patients after total knee arthroplasty,and the main influencing factors include pain,function,activity degree,muscle strength,flexion deformity,stability and stiffness degree.
