摘　　要：目的:分析基于互联网平台的闭环式健康管理对腹膜透析患者生活质量的影响。方法:构建腹膜透析健康管理平台,组建腹膜透析健康管理团队,选取2017年3月至2018年3月在河池市人民医院肾内科行腹膜透析患者136例,按住院号单、双分为观察组和对照组各68例。对照组给予腹膜透析常规护理,观察组在常规护理基础上,实施院内、院外闭环式健康管理模式,根据营养不良-炎症评分(MIS评分)和SF-36健康调查简表,比较两组患者出院后3个月的健康状况、各生化指标、生活质量总指标及并发症等。结果:观察组的MIS评分明显低于对照组,观察组的体质量指数(BMI)、血清白蛋白(ALB)、血红蛋白(Hb)及生活质量总指标得分均明显高于对照组;而观察组的腹膜炎、隧道口感染、引流不畅等发生率均低于对照组,差异均有统计学意义(P<0.05)。结论:基于互联网平台的闭环式健康管理能有效改善腹膜透析患者的营养状况,减少并发症发生率,提高生活质量。Objective: To study the effects on life quality in patients with peritoneal dialysis using closed-loop health management based on internet platform. Methods: Construct a peritoneal dialysis health management platform and establish a peritoneal dialysis health management team.From March 2017 to March 2018, 136 cases of peritoneal dialysis patients were selected in the department of nephrology in our hospital. These patients were divided into to observation group and control group by their inpatient numbers in single and double, respectively. Patients in the control group were cared by routine nursing, while patients in the other group were treated by adding closed-loop health management to the basis of routine nursing. Patients from two groups were compared by malnutration inflammation score(MIS) and the MOS item short from health survey(SF-36) in health condition, biochemical indexes, life quality scores from all dimensions and complications after three months` discharge. Results: The MIS scores in observation group were obviously lower than the control group, patients in the observation group got higher scores than in control group in all kinds of biochemical indexes, such as Body Mass Index(BMI), Serum albumin(ALB), Hemoglobin( Hb), and various dimensions` life quality scores. In contrary, the occurrence rates of peritonitis, tunnel front infection, inadequate drainage in patients from the observation group were lower than that of control group, presenting a statistical difference(P<0.05). Conclusion: Closed-loop Health Management Based on Internet Platform could be helpful in improving the nutritious status in patients with peritoneal dialysis and decreasing the incidence of complications so as to improve their life quality.
