Ⅰ期融合术对伴第一跖跗关节脱位Lisfranc损伤患者疼痛程度及生活质量的影响

摘　　要：目的研究Ⅰ期融合术对伴第一跖跗关节脱位Lisfranc损伤患者疼痛程度及生活质量的影响。方法选取某院2017年12月至2018年7月收治的伴第一跖跗关节脱位Lisfranc损伤患者88例作为研究对象,随机数表法分为两组。对照组行切开复位内固定法治疗,观察组行Ⅰ期融合术治疗,比较两组患者术后疼痛情况及随访6个月后的生活质量。结果观察组术后2 h、3 d及出院前的视觉疼痛模拟评分(visual analogue scale,VAS)均低于对照组,差异有统计学意义(P<0.05);术后观察组的生活质量调查表(Short Form-36,SF-36)各项评分优于对照组,差异有统计学意义(P<0.05);术后观察组并发症的总发生率(4.55%)比对照组(20.45%)低,差异有统计学意义(P<0.05)。结论Ⅰ期融合术治疗伴第一跖跗关节脱位Lisfranc损伤患者,可有效缓解患者的术后疼痛感,降低术后并发症的发生率,提高患者术后生活质量。Objective To study the effect ofⅠ-stage fusion on pain degree and quality of life in Lisfranc injury patients with first tarsometatarsal joint dislocation.Methods 88 Lisfranc injury patients with first tarsometatarsal joint dislocation admitted in the hospital from December 2017 to July 2018 were selected as the study objects,who were divided into control group and observation group according to random number table,with 44 cases in each group.The control group underwent open reduction and internal fixation,while the observation group was treated withⅠ-stage fusion.The postoperative pain and quality of life after follow-up for 6 months were compared between the two groups.Results After surgery for 2 h and 3 d and before discharge,the scores of VAS(visual analogue scale)in the observation group were lower than those in the control group,with difference of statistical significance(P<0.05);After surgery,the scores of SF-36 scale in the observation group were better than those in the control group,with difference of statistical significance(P<0.05);The total incidence of postoperative complications in the observation group(4.55%)was lower than that in the control group(20.45%),with difference of statistical significance(P<0.05).ConclusionⅠ-stage fusion in the treatment of Lisfranc injury patients with first tarsometatarsal joint dislocation can effectively relieve postoperative pain,decrease the incidence of postoperative complications,and increase postoperative quality of life.
