摘 要:目的:分析胸腔镜下R3(第3肋骨表面交感神经主干)离断术中患者掌温变化,探讨iThermonitor体温传感器对手汗症患者术中掌温监测的应用效果.方法:选择原发性手汗症患者1例,在全麻下行胸腔镜左右侧R3离断术,采用iThermonitor体温传感器对患者手掌和腋窝深部体温进行监测.结果:术前体温监测结果:患者左、右手掌、腋下体温为(36.2±0.02)?C、(36.1±0.03)?C、(36.7±0.21)?C;术中离断左、右侧R3后5、10min内的掌温与离断前相比,差异有统计学意义(P<0.05);但是未发生明显体温升高(≥1~3?C);秩相关分析显示腋下体温与掌温呈正相关.结论:iThermonitor可以为手汗症患者提供连续、动态的术中体温监测,但掌温上升幅度是否与该手术效果相关仍待更多的研究予以验证.Objective:To analyze the changes in palm temperature of patients undergoing thoracoscopic R3(3rd rib surface sympathetic trunk)surgery,and to explore the application effect of iThermonitor body temperature sensor on palm temperature monitoring during hand sweat surgery.Methods:One patient with primary hand sweat was selected,and the left and right sides of the thoracoscope were separated by R3 under general anesthesia.The iThermonitor temperature sensor was used to monitor the body temperature of the palms and axilla of the patient.Results:Preoperative body temperature monitoring results:The left and right palm and axillary body temperature of the patients were(36.2±0.02)?C,(36.1±0.03)?C,(36.7±0.21)?C;the palms within 5 and 10 minutes after the left and right R3 were disconnected during the operation There was a statistically significant difference in temperature between before and after separation(P<0.05);however,no significant increase in body temperature(≥1~3?C)occurred.Rank correlation analysis showed a positive correlation between axillary body temperature and palm temperature.Conclusion:iThermonitor can provide continuous and dynamic intraoperative body temperature monitoring for patients with hand sweat,but the relevance of this surgical effect remains to be verified by more studies.
