经验穴“五关”刺络放血联合头针治疗中风后手指拘挛性瘫痪疗效观察

摘　　要：目的观察经验穴“五关”刺络放血联合头针治疗中风后手指拘挛性瘫痪的临床疗效。方法将60例中风后手指拘挛性瘫痪患者按随机数字表分为两组,每组30例。治疗组采用经验穴“五关”刺络放血联合头针治疗,对照组采用常规康复治疗。比较两组治疗前后手指拘挛程度评分、手指肌力评分、前臂屈伸肌群表面肌电信号及总体疗效的差异。结果治疗组总有效率为93.3%,对照组总有效率为83.3%,两组比较差异有统计学意义(P<0.05);两组治疗后手指拘挛程度评分及手指肌力评分比较差异具有统计学意义(P<0.05),前臂屈伸肌群表面肌电信号比较差异具有统计学意义(P<0.05)。结论经验穴“五关”刺络放血联合头针治疗中风后手指拘挛性瘫痪疗效明显,在降低拘挛手指肌张力和提高其肌力方面具有相应优势。Objective To observe the clinical efficacy of collateral-pricking bloodletting at the empiric points Wu Guan plus scalp acupuncture in treating post-stroke spastic paralysis of fingers.Method Sixty patients with post-stroke spastic paralysis of fingers were divided into two groups by the random number table,with 30 cases in each group.The treatment group was intervened by collateral-pricking bloodletting at the empiric points Wu Guan,and the control group was treated with conventional rehabilitation.The two groups were compared with each other in terms of the spasticity degree score,finger muscle strength score,the surface electromyography(sEMG)signals of the flexor and extensor muscles of forearm and the general therapeutic efficacy before and after treatment.Result The total effective rate was 93.3%in the treatment group versus 83.3%in the control group,and the between-group difference was statistically significant(P<0.05).After treatment,there were significant differences comparing the finger spasticity degree score and muscle strength score between the two groups(P<0.05),as well as the sEMG signals of the flexor and extensor muscles of forearm(P<0.05).Conclusion Collateral-pricking bloodletting at the empiric points Wu Guan combined with scalp acupuncture can produce significant efficacy in treating post-stroke spastic paralysis of fingers,and it shows certain advantages in reducing the muscle tension and increasing the muscle strength of the spastic fingers.
