摘　　要：目的了解昆山市65岁以上老年居民代谢综合征(MS)现状与特点,为其管理提供科学依据.方法抽样调查昆山市4个镇,20个村(社区)65岁以上常住老年居民21930名,依照2005国际糖尿病联盟MS标准诊断,调查MS流行情况并分析各组分如肥胖、高血压、高血脂、高血糖患病情况,分析相关高危因素.结果65岁以上老年人MS患病率为29.8%,女性(44.6%)高于男性(13.6%);MS各组分患病率比较显示,高血压的患病率最高,为55.7%,其次为肥胖、高甘油三酯、低高密度脂蛋白;男女组间比较,高血糖及低高密度脂蛋白血症女性患病率均显著高于男性;Logistic多元逐步回归分析显示,高血糖、高血压、高血脂、高尿酸血症是MS主要影响因素,饮酒、肉食饮食和身体活动水平有部分与MS相关.结论昆山市65岁以上老年居民MS发病率女性高于男性.鉴于MS在65岁以上老年居民中的高发病率,应重视MS的预防、治疗和健康宣教,对高危人群应定期体检,尽早诊治.同时应进行生活方式于预,如增加运动和合理膳食等,提高生活质量.Objective To investigate the present situation and characteristics of metabolic syndrome(MS)in elderly residents over 65 years old in Kunshan City,and to provide scientific basis for its management.Methods A sample survey was conducted among 21930 residents over 65 years old in 4 towns and 20 villages(communities)of Kunshan City.MS was diagnosed by 2005 International Diabetes Union MS standard.The prevalence of MS was investigated and the prevalence of obesity,hypertension,hyperlipidemia and hyperglycemia were analyzed,and the related high risk factors were analyzed.Results The prevalence rate of MS in elderly over 65 years old was 29.8%,44.6%in female was higher than that in male 13.6%.The prevalence rate of hypertension was the highest(55.7%),followed by obesity,high triglyceride,low high density lipoprotein,and the prevalence of hyperglycemia and low high density lipoprotein in women was significantly higher than that in male.Log is tic multiple stepwise regression analysis showed that hyperglycemia,hypertension,hyperlipidemia and hyperuricemia were the main influencing factors of MS.Drinking,meat eating and physical activity were partly related to MS.Conclusion The incidence of MS in elderly residents over 65 years old in Kunshan City is higher in females than in males.In view of the high incidence of MS among elderly residents over 65 years old,attention should be paid to the prevention,treatment and health education of MS,and the high risk population should be examined regularly and diagnosed and treated as soon as possible.At the same time,we should carry out lifestyle prediction,such as increasing exercise and reasonable diet,etc.,to improve the quality of life.
