摘　　要：目的评价护理干预对Revolution CT冠状动脉CT血管造影(computed tomography angiography,CTA)图像质量的影响.方法选取苏州大学附属第一医院2018年1月-2019年6月采用Revolution CT行冠脉CTA检查的220例患者,随机分为护理干预组(n=110)和对照组(n=110),对照组仅进行常规护理,护理干预组则同时进行心理护理、心率控制和呼吸屏气训练等.对重建出的冠状CTA图像质量进行主观评分和客观指标评价,计算并比较两组患者右冠、左冠前降支及回旋支的信噪比(signal-to-noise ratio,SNR)、对比噪声比(contrast-to-noise ratio,CNR).结果护理干预组中评分为3分和4分的比例(74.55%)明显高于对照组(49.09%),两组患者冠脉CTA图像质量主观评分的差异有统计学意义(P=0.001).护理干预组的客观评价指标SNR右冠、CNR右冠、SNR前降支、CNR前降支、SNR回旋支和CNR回旋支均大于对照组,差异均有统计学意义(P均<0.001).结果表明护理干预组的图像质量明显优于对照组.结论护理干预可明显提高Revolution CT冠脉CTA图像质量,在采用Revolution CT行冠脉CTA检查时进行护理干预有重要意义.Objective To evaluate the effect of nursing intervention on image quality of computed tomography angiography(CTA)of coronary artery by Revolution CT.Methods 220 patients who underwent CTA examination of the coronary artery by Revolution CT in the First Affiliated Hospital of Soochow University from January 2018 to June 2019 were randomly divided into nursing intervention group(n=110)and control group(n=110).The control group only received routine nursing,while the nursing intervention group received psychological nursing,heart rate control and breath holding training at the same time.The image quality of reconstructed coronary artery CTA was evaluated by subjective score and objective index.The signal-to-noise ratio(SNR)and contrast-to-noise ratio(CNR)of right coronary artery,left anterior descending coronary artery and circumflex coronary artery were calculated and compared between the two groups.Results The proportion of score 3 and 4 in nursing intervention group(74.55%)was significantly higher than that in control group(49.09%).There was significant difference in subjective score of coronary artery CTA image quality between the two groups(P=0.001).The objective indexes of SNRrca,CNRrca,SNRLAD,CNRLAD,SNRLCX and CNRlcx in nursing intervention group were higher than those in control group,with statistical significance(P<0.001).The results showed that the image quality of nursing intervention group was better than that of control group.Conclusion Nursing intervention can significantly improve the image quality of coronary artery CTA by Revolution CT.Nursing intervention is of great significance in the examination of coronary artery CTA by Revolution CT.
